dc.description.abstract | HINTERGRUND: Gastrointestinale Stromatumoren (GIST) sind CD117-positive mesenchymale Tumoren des Gastrointestinaltrakts. Mit molekularzytogenetischen Methoden konnten in der letzten Dekade einige Zusammenhänge von chromosomalen Verlusten (u.a. -14q, -1p und -22q) mit der Prognose der betroffenen Patienten aufgezeigt werden. Als tumorauslösendes Ereignis gilt dabei eine Mutation des KIT- oder PDGFRA-Gens.ZIEL: Das Ziel der vorliegenden Arbeit ist die Korrelation der Ergebnisse einer komparativen genomischen Hybridisierung (CGH) mit den Ergebnissen einer Mutationsanalyse des KIT- bzw. PDGFRA-Gens.METHODEN: 64 Tumorproben von GIST aus dem Zeitraum von 1993 - 2005 wurden mittels CGH auf chromosomale Imbalanzen (CI) untersucht. Zusätzlich wurde eine Mutationsanalyse des KIT-Gens (Exon 9, 11, 13 und 17) sowie des PDGFRA-Gens (Exon 18 und 12) nach dem Sanger-Verfahren durchgeführt.ERGEBNISSE: GIST mit einer Mutation in KIT Exon 11 Codon 557 / 558 zeigen im Vergleich zu Tumoren mit Mutation und >= 5 CI ein signifikanteres Ergebnis bzgl. der rezidivfreien Überlebenszeit (p = 0,028 vs. p < 0,001). Bei GIST mit Mutation im PDGFRA-Gen (mit einer signifikant längeren rezidivfreien Überlebenszeit) geht bei den Tumoren >= 5 CI das Signifikanzniveau verloren (p = 0,037 vs. p = 0,169). Tumoren mit einem chromosomalen Verlust an 14q oder 1p zeigen keinen Zusammenhang mit der rezidivfreien Überlebenszeit (p = 0,231 bzw. p = 0,067). Bei Betrachtung der Tumoren mit der CI und Mutation in KIT Exon 11 Codon 557 / 558 zeigt sich bei einem Verlust an 1p eine signifikant kürzere rezidivfreie Überlebenszeit (p = 0,001). Bei einem Verlust an 14q und dieser Mutation in Kit Exon 11 wird das Signifikanzniveau nur knapp verfehlt (p = 0,055). Bei einem Verlust an 22q wird sowohl mit als auch ohne Mutation ein signifikantes Ergebnis erreicht (p = 0,045 vs. p = 0,001). | de |

With molecular-cytogenetic methods several relationships between DNA copy number losses (i.e. -14q, -1p and -22q) and the prognosis of the affected patients could be shown in the last decade. A mutation of the KIT- or PDGFRA-Gene is believed to be the tumor-initiating event.OBJECTIVES: The goal of the present paper is the correlation of the results of a comparative genomic hybridization (CGH) and a mutational analysis of the KIT- and PDGFRA-Gene.METHODS: 64 GISTs from 1993 - 2005 were examined for chromosomal imbalances (CI) by CGH. Additional, a mutational analysis of the KIT-Gene (Exon 9, 11, 13 and 17) and the PDGFRA-Gene (Exon 18 and 12) was perfomed via Sanger sequencing.RESULTS: GISTs with a mutation in KIT Exon 11 Codon 557 / 558 in comparison to tumors with a mutation and >= 5 CI reveal significant results for the recurrence free survival (p = 0.028 vs. p < 0.001). GISTs with a mutation in the PDGFRA-Gene (and a significant longer recurrence free survival) loses their significance niveau for tumors with a mutation and >= 5 CI (p = 0.037 vs. p = 0.169). Tumors with DNA copy number losses on 14q or 1p reveal no relationship with the recurrence free survival (p = 0.231 bzw. p = 0.067). Tumors with the CI and a mutation in KIT Exon 11 Codon 557 / 558 reveal a significant shorter recurrence free survival for DNA copy number losses on 1p (p = 0.001). The significance niveau is shortly missed for a loss on 14q and this mutation in KIT Exon 11 (p = 0.055). With and without this mutation the result for the recurrence free survival is significant for a loss on 22q (p = 0.045 vs. p = 0.001). | de |
