Cigarette smoking is the leading cause of preventable death in the western world and the positive effect of smoking cessation on morbidity and mortality is well known. Endothelial progenitor cells (EPC) participate in angiogenesis and vascular repair and cardiovascular risk factors may reduce their numbers or impair their functional properties.The present paper examines for the first time the effect of smoking cessation on the functional properties of EPC and enlarges on earlier examinations, which found an unchanged number of circulating CD34+-VEGF-R2+-double positive cells and simultaneously a lower number of cultured acLDL+-lectin+cells after successful smoking cessation (Puls et al. 2011). Following the hypothesis of a modified adhesion of the EPC, experiments concerning the adhesion properties and the expression of integrins as well as the existence of oxidative stress were performed. The studies were carried out on EPC of a heterogeneous group of individuals, who successfully had completed a 5-week smoking cessation programme. To this end cells were isolated from the peripheral blood and cultured on fibronectin for seven days. The present dissertation shows that smoking cessation is a associated with significant reduced EPC adhesion to fibronectin or TNFα-activated endothelial cells, and a diminished incorporation of EPC into endothelial cell networks. Futhermore, significantly reduced β1- and β2-integrin expression and lower contents of intracellular reactive oxygen species (ROS) were detected in EPC following smoking cessation, in addition to reduced plasma asymmetric dimethyl-L-arginine (ADMA) levels. It is important to underline the fact, that the cells of an individual were analysed before and after smoking cessation, not those of smokers and non-smokers. Obviously there is a difference in the endothelial physiology of former smokers and lifelong non-smokers. A reduction of low-grade inflammation or alternatively oxidative stress after smoking cessation could play an important role on the influence of adhesion properties of EPC.

vermeidbarer Todesfälle in der westlichen Welt und der positive
Einfluss von Tabakentwöhnung auf Morbidität und Mortalität ist
bekannt. Endotheliale Progenitorzellen (EPC) sind an Reparatur- und
Angiogeneseprozessen in den Gefäßen beteiligt und können durch
kardiovaskuläre Risikofaktoren in Anzahl und Funktion beeinflusst
werden. Die vorliegende Arbeit untersucht nun erstmalig den
Einfluss von Tabakentwöhnung auf die funktionellen Eigenschaften
von EPC und baut damit auf vorangegangenen Untersuchungen auf, die
bei gleicher Anzahl zirkulierender CD34+-VEGF-R2+- doppelt
positiver Zellen eine reduzierte Anzahl kultivierbarer
acLDL+-lectin+-Zellen nach erfolgreicher Tabakentwöhnung zeigten
(Puls et al. 2011). Es wurde der Hypothese nachgegangen, dass ein
verändertes Adhäsionsverhalten der EPC zugrunde liegen könnte. Dazu
wurden Versuche zum Adhäsionsverhalten der Zellen durchgeführt und
die Expression von Integrinen sowie das Vorliegen von oxidativem
Stress analysiert.Die Untersuchungen wurden an EPC einer
heterogenen Probandengruppe, die erfolgreich an einem fünfwöchigen
Tabakentwöhnungsprogramm teilnahm, durchgeführt. Dafür wurden die
Zellen zunächst aus dem peripheren Blut isoliert und für sieben
Tage auf Fibronektin kultiviert. Diese Arbeit zeigt, dass
Tabakentwöhnung mit einem signifikant verminderten
Adhäsionsvermögen von kultivierten EPC an Fibronektin und
TNF-α-stimulierten HUVEC (Human Umbilical Vein Endothelial Cells)
einhergeht. Zudem war die Anzahl inkorporierender EPC in
kapillarähnlichen Strukturen reduziert. Des Weiteren führte
Tabakentwöhnung zu einer signifikant verminderten Expression von
β1- und β2-Integrinuntereinheiten auf den EPC, zu erniedrigten
Level von intrazellulärem ROS (reactive oxygen species) in den EPC
sowie zu geringeren ADMA-Spiegeln (Asymmetrisches Dimethylarginin)
im Plasma der Probanden.Es ist zu betonen, dass in der vorliegenden
Arbeit die Zellen eines Individuums vor und nach Tabakentwöhnung
miteinander ver glichen wurden und nicht die von Rauchern und
Nichtrauchern. Offenbar unterscheiden sich die endotheliale
Physiologie bzw. die Eigenschaften von EPC von ehemaligen Rauchern
und lebenslangen Nichtrauchern. Eine wichtige Rolle könnten dabei
die durch Tabakentwöhnung reduzierte low-grade- Entzündung bzw.
oxidativer Stress und deren Einfluss auf das Adhäsionsverhalten der
EPC spielen.
