The alport syndrome is the second most common hereditary renal disease, that is characterized by chronic and progressive renal failure. The main therapeutic focus at present is to delay the progression of the disease, since the renal transplantation is the only effective therapeutic strategy available. The blockade of the local angiotensin system by means of ACE inhibitors or AT1 receptor antagonists (AT1RA) represents a promising attempt to delay the progression of alport nephritis and the accompanying tubulointerstitial fibrosis. In the present study, the author using the animal model of the collagen4A3 deficient mice (COL4A3-/-) examines the effectiveness of ACE inhibitor Enalapril (EN) and AT1RA Irbesartan (IB) in delaying the progression of the chronic renal insufficiency. Furthermore, the diminished degradation of interstitial matrix components by the plasminogen plasmin system contributes to the disease progression, besides the increased production of extracellular matrix in tubulointerstitial fibrosis. In contrast to reduction of progression of renal fibrosis the Bone Morphogenetic Protein 7 (BMP7), a morphogen of TGFβ superfamily, has the ability to stop renal fibrosis completely and even reverse it. Therefore the expression of Plasminogen Activator Inhibitor 1 (PAI1), BMP7 and its receptor ALK3 in the context of the alport nephritis and under medication with Enalapril and Irbesartan was examined additionally. COL4A3-/- animals were treated with 10 mg/kg KG Enalapril (n = 14), 50 mg/kg KG Irbesartan (n = 12) or placebo (n = 17), sacrificed after 70 days and tissue samples were harvested. Kidney function was determined using the creatinine and blood urea nitrogen (BUN). Siriusred staining and hydroxyproline assay served for the quantification of renal fibrosis. Expression analysis were carried out by means of RT-PCR, immunohistochemistry and immunofluorescence. In comparison with untreated COL4A3-/- animals, the treated animals (EN-COL4A3-/- and IB-COL4A3-/-) show a significantly lower concentration of the retention values of creatinine and BUN in the serum, a less distinctive tubulointerstitial proliferation, and tubulointerstitial fibrosis with a lower accumulation of alpha smooth muscle actin and FSP1 expressing fibroblasts in which Irbesartan shows a stronger effect. COL4A3-/- animals display a PAI1 overexpression which is considerably less distinct in treated animals. Furthermore, COL4A3-/- animals in comparison with wildtype animals show a significant reduction of the BMP7 expression in the renal parenchyma, whereas this difference is diminished after Enalapril medication but not with Irbesartan medication. Both EN-COL4A3-/- and IB-COL4A3-/- animals show an increased expression of the BMP7 receptor ALK3 compared to wildtype and untreated COL4A3-/- animals. The present study confirms that the blockade of the local angiotensin system leads to the delay of progression of the chronic progressive renal insufficiency in the context of alport nephritis where the AT1RA Irbesartan shows stronger effects than the ACE inhibitor Enalapril. The question which of the two drug classes is more suitable for general treatment of chronic renal insufficiency cannot be answered by the present study. Also the relevance of less reduced BMP7 expression, the ALK3 overexpression and the lowered PAI1 expression by RAS blockade, for the reduction of progression of the tubulointerstitial fibrosis is not completely resolved.

Das Alport-Syndrom ist die zweithäufigste
Ursache einer hereditären chronisch progredienten
Niereninsuffizienz, wobei derzeit in der Behandlung die
Progressionsminderung der Erkrankung im Vordergrund steht, da eine
kausale Therapie nur mittels Nierentransplantation möglich ist. Die
Blockade des lokalen Angiotensinsystems mittels ACE-Hemmer oder
AT1-Rezeptor-Antagonisten (AT1RA) stellt einen vielversprechenden
Ansatz zur Progressionsminderung der Alport-Nephritis und der damit
einhergehenden tubulointerstitiellen Fibrosierung dar. In der
vorliegenden Studie untersucht der Autor im Tiermodell der
Kollagen4A3-defizienten Mäuse (COL4A3-/-) den ACE-Hemmer Enalapril
(EN) und den AT1RA Irbesartan (IB) hinsichtlich ihrer Wirksamkeit
die Progression der chronischen Niereninsuffizienz zu verzögern.
Des Weiteren spielt neben der vermehrten Produktion interstitieller
Matrixbestandteile der verminderte Abbau derselben durch das
Plasminogen-Plasmin-System bei der tubulointerstitiellen
Fibrosierung eine entscheidende Rolle. Im Gegensatz zur
Progressionsminderung der renalen Fibrosierung ist das Bone
Morphogenetic Protein-7 (BMP-7), ein Morphogen aus der
TGF-β-Superfamilie, in der Lage, die renale Fibrosierung gänzlich
zu stoppen und sogar umzukehren. Daher wird zusätzlich die
Expression des Plasminogen-Aktivator-Inhibitors (PAI-1), des BMP-7
und dessen Rezeptors ALK-3 im Rahmen der Alport-Nephritis und unter
Medikation mit Enalapril bzw. Irbesartan untersucht.
COL4A3-/--Tiere wurden mit 10 mg/kg KG Enalapril (n=14), 50 mg/kg
KG Irbesartan (n=12) oder Placebo (n=17) behandelt und nach 70
Tagen getötet. Die Nierenfunktion wurde anhand der Kreatinin- und
Harnstoff-N-Konzentration im Serum bestimmt. Zur Quantifizierung
der renalen Fibrosierung dienten die Pikrosiriusrot-Färbung und der
Hydroxy-Prolin-Assay. Die Expressionsanalysen erfolgten mittels
RT-PCR, Immunhistochemie und Immunfluoreszenz. EN- und
IB-COL4A3-/--Tiere zeigen im Vergleich zu unbehandelten
COL4A3-/--Tieren eine signifikant niedrigere Konzentration der
Retentionswerte Kreatinin und Harnstoff-N im Serum und weisen eine
weniger stark ausgeprägte tubulointerstitielle Proliferation und
Fibrosierung mit geringerer Akkumulation Alpha-Glattmuskel-Aktin
und FSP-1 exprimierender Fibroblasten auf, wobei Irbesartan im
Vergleich zu Enalapril einen stärkeren Effekt zeigt. Bei
COL4A3-/--Tieren kommt es zu einer PAI-1-Überexpression, welche bei
den behandelten COL4A3-/--Tieren deutlich geringer ausgeprägt ist.
Des Weiteren zeigen COL4A3-/--Tiere im Vergleich zum Wildtyp eine
signifikante Verminderung der BMP-7-Expression im Nierenparenchym,
wobei dieser Unterschied nach Enalapril-Medikation vermindert und
nach Irbesartan-Medikation nicht zu finden ist. Sowohl
EN-COL4A3-/-- als auch IB-COL4A3-/--Tiere weisen im Vergleich zu
unbehandelten COL4A3-/--Tieren als auch zum Wildtyp eine erhöhte
Expression des BMP-7-Rezeptors ALK-3 auf. In der vorliegenden
Studie wird im Tierexperiment bestätigt, dass die Blockade des
lokalen Angiotensinsystems zur Progressionsminderung der chronisch
progredienten Niereninsuffizienz im Rahmen der Alport-Nephritis
führt, wobei der AT1RA Irbesartan stärkere Effekte zeigt als der
ACE-Hemmer Enalapril. Die Frage, welche der beiden Substanzklassen
für die Behandlung einer chronisch progredienten Niereninsuffizienz
generell besser geeignet ist, lässt sich anhand der vorliegenden
Studie allerdings nicht beantworten. Die Relevanz der mit der
RAS-Blockade assoziierten weniger stark ausgeprägten Abnahme der
BMP-7-Expression, der ALK-3-Überexpression und der geringeren
PAI-1-Expression für die Progressionsminderung der
tubulointerstitiellen Fibrosierung ist noch nicht vollständig
geklärt.
