Bacterial meningitis is still a quite severe illness which is associated with local and systemic inflammation and can cause long-term neurological sequelae or death. Common bacteria causing meningitis in Germany are S. pneumoniae, E. coli, N. meningitidis and L. monocytogenes; the current guide line associating antibiotic treatment contains beta-lactam antibiotics such as penicillin G or cephalosporine like ceftriaxone; these antibiotics induce bacteriolysis, inhibit cell wall synthesis and rapidly sterilize the cerebrospinal fluid; the release of high amounts of cell wall compounds, bacterial DNA and toxines has high proinflammatory potency and is a strong stimulator for the immune system. In succession there is an inflammatory host response with a strong burst of inflammation and tissue damage, followed by a worsening of the clinical symptoms; therefore the aim is to reduce the release of proinflammatory and toxic compounds and thereby to minimize neuronal damage. We evaluated a concept with a short-term pretreatment with the protein synthesis-inhibiting antibiotic rifampicin; this antibiotic was dispensed only one hour before starting the standard antibiotic therapy with ceftriaxone. First we evaluated this concept in vitro by logarithmically growing pneumococcal cultures exposed to different antibiotics; afterwards rabbits, suffering from pneumococcal meningitis, were treated by ceftriaxone alone or in combination with rifampicin. In conclusion we detected that a short-term pretreatment with rifampicin reduced the release of bacterial toxic compounds in vitro and in vivo. An attenuated inflammation and thereby a decreased neuronal cell loss followed by less clinical symptoms and neurological sequelae might be the result. Therefore this concept has the potential of a high weight in the therapy of meningitis and should be subject further clinical trials.

Eine bakterielle Meningitis stellt nach
wie vor eine schwere Erkrankung dar, die mit lokaler und
systemischer Entzündungsreaktion einhergeht und zu neurologischen
Spätkomplikationen bis hin zum Tod führen kann. In Deutschland
übliche Erreger sind insbesondere S. pneumoniae, E. coli, N.
meningitidis und L. monocytogenes; die derzeitige Leitlinien
gerechte Therapie sieht die Behandlung mit ß-Laktam Antibiotika wie
Penicillin G oder einem Cephalosporin wie Ceftriaxon vor; diese
Antibiotika bewirken eine Bakteriolyse, inhibieren die
Zellwand-Synthese und führen zu einer schnellen Sterilisierung des
cerebralen Liquors; die hohe Freisetzung von Zellwandkomponenten,
bakterieller DNA und Toxinen wirkt proinflammatorisch und ist ein
starker Stimulus für das Immunsystem.
In der Folge kommt es zu einer Immunreaktion mit einem
Entzündungsanstieg und Gewebeschaden, gefolgt von einer
Verschlechterung der klinischen Symptome; daher ist es das Ziel die
Freisetzung proinflammatorischer und toxischer Komponenten zu
reduzieren und dadurch den Zellschaden zu minimieren. Wir
erarbeiteten ein Schema, das die kurzzeitige Vorbehandlung mit dem
Proteinsynthese hemmendem Antibiotikum Rifampicin vorsieht;
Rifampicin wurde eine Stunde vor Beginn der Standardantibiotika
Therapie mit Ceftriaxon verabreicht. Zuerst wurde dieses
Behandlungsschema in vitro anhand von Pneumokokken Kulturen, die
sich in der logarithmischen Wachstumsphase befanden und
verschiedenen Antibiotika ausgesetzt wurden, getestet; anschließend
wurden Kaninchen, die an einer Pneumokokken Meningitis erkrankt
waren, entweder nur mit Ceftriaxon oder in Kombination mit
Rifampicin behandelt. Zusammenfassend konnte festgestellt werden,
dass eine kurze Vorbehandlung mit Rifampicin die Freisetzung
bakterieller Komponenten in vitro und in vivo reduziert. Eine
verminderte Entzündungsreaktion und dadurch ein verringerter
neuronaler Zellschaden, gefolgt von einer geringeren klinischen
Beschwerdesymptomatik und weniger neurologischen Spätfolgen dürfte
damit erreicht werden.
Daher hat dieses Konzept Potential einen hohen Stellenwert in der
Therapie der Meningitis zu erreichen und sollte daher Gegenstand
weiterer klinischer Forschung sein.
