Objective There is evidence that diets with a high proportion of typical omega 3 polyunsaturated fatty acids (n-3 PUFAs) can be effective for weight management and prevention of cardiovascular disease. For example there are a lot of n-3 PUFAs in walnuts and canola oil. Therefore, a modified diet using fat modification through neutral and butterflavored canola oil, walnuts und walnut oil was tested on the effect of weight management and prevention of cardiovascular disease. Two portion-controlled sweet daily snacks were also included in the diet. Subjects/Methods It was a randomized waiting-list control study with overweight/ grade 1 obese subjects. The first 12-weeks were a self-help modified Mediterranean type diet, 6 weeks of dietplans and 6 weeks of weight loss maintenance training. The trial duration was 12 months. Intervention group (IG): 100 participants ( average age of 52.4 years, weight 85.1kg, BMI 30.1 kg/m²), waiting-list control group (CG): 112 participants (52.6 years, 84.1kg, 30.1 kg/m²). Results Per protocol weight loss after 12 weeks was 5.2kg in IG vs. 0.4kg in CG (p≤0.0001), BMI -1.8 kg/m² vs. -0.1 kg/m² (p≤0.0001), waist circumference -4.7cm vs. -0.9cm (p≤0.0001). Triglycerides, total cholesterol and LDL-cholesterol improved significantly in IG but not in CG. Dropouts: 28 % in IG, 69 % in CG. Weight loss after 12 months: 4.2kg (pooled data). Conclusion Our modified Mediterranean type diet using fat modification through neutral und butterflavored canola oil, walnuts and walnut oil with two daily portion-controlled sweet snacks was effective for weight management and improved blood lipids even at 12 month. The diet was testet in a self-help setting for overweight and grade 1 obese subjects.
