Intraoperative ultrasound is able to display dynamic processes intraoperatively in real-time fashion. So in case of burr-hole biopsies it seems to be an interesting option for neurosurgeons. Value and complications of this technique must be evaluated in a larger series. One hundred consecutive intracranial biopsies were analyzed. Through a burr hole, the lesion was localized by ultrasonography, and the planned needle trajectory was superimposed onto the image. Intracranial vessels were imaged by Doppler flow signals. Biopsies were taken in a mean depth of 4.1 cm from different parts of each tumor. Forty nine lesions were located deeply within the white matter. There were 18 temporal and three cerebellar lesions. The mean operative time was 45 minutes. In 92% of the lesions, a specific histopathologic diagnosis could be established. Postoperative neurologic deficits occurred in eight patients, three of them were persisting. In comparison with stereotactic biopsies the technique of ultrasound-guided biopsies revealed a similar percentage of conclusive diagnoses and complication rate.

Intraoperativer Ultraschall ermöglicht die dynamische Darstellung intraoperativer Prozesse in Echtzeit. Daher stellt die Ultraschall-gestützte Bohrloch-Biopsie eine interessante Option für Neurochirurgen dar. Der Stellenwert der Methode bezüglich Rate an relevanten Ergebnissen und Komplikationen ist nicht klar und bedarf einer Evaluation an einem größeren Patientenkollektiv. Insofern erfolgte die systematische Analyse von 100 konsekutiven intrakraniellen Biopsien. Über ein Bohrloch erfolgte die Lokalisation der Läsion mittels Ultraschall, die geplante Nadel-Trajektorie wurde auf das Ultraschallbild projiziert. Hierbei wurden intrakranielle Gefäße mittels Doppler-Sonographie dargestellt. Die Biopsien wurden aus einer durchschnittlichen Tiefe von 4,1 cm aus mehreren Lokalisationen im Tumor entnommen. Neunundvierzig Läsionen waren in den tiefen Strukturen des Hirns lokalisiert. In 18 Fällen lagen temporale Prozesse vor, bei 3 Patienten fand sich die Läsion infratentoriell. Die durchschnittliche Operationszeit betrug 45 Minuten. In 92% der Fälle gelang es eine aussagekräftige, histopathologische Diagnose zu sichern. Postoperativ traten bei acht Patienten neurologische Defizite auf, in 3 Fällen persistierten diese. Im Vergleich zur stereotaktischen Biopsie bietet die Technik der Ultraschall-gestützten Biopsie eine vergleichbare Rate bezüglich Treffsicherheit und Komplikationen.
