dc.description.abstractger | Neuroendokrine Tumoren des Thymus sind sehr seltene Neoplasien. Als nächstverwandte
Tumorgruppe gelten die neuroendokrinen Tumoren der Lunge. Beide werden nach den
gleichen Kriterien in die vier Subtypen typisches Karzinoid, atypisches Karzinoid, großzelliges
neuroendokrines Karzinom und kleinzelliges (Lungen-)Karzinom eingeteilt. Gleichwohl
bestehen zwischen den Tumorgruppen auch einige Unterschiede, beispielsweise die
relative Häufigkeit der Subgruppen, die Geschlechterverteilung und die biologische Aggressivität.
Die neuroendokrinen Thymustumoren haben vor allem bei den Karzinoiden eine
schlechtere Prognose. Um das bislang vergleichsweise geringe Wissen über diese seltenen
Tumoren zu erweitern, wurde in dieser Arbeit eine quantitative massenspektrometrischproteomische
Analyse durchgeführt. Der hierzu spezifisch hergestellte Proteinquantifizierungsstandard
ermöglicht zum einen eine zukünftige Analyse von weiteren neuroendokrinen
Tumoren des Thymus und der Lunge und zum anderen suggerieren die erhobenen
Daten, dass er auch für die Analyse anderer neuroendokriner Tumorentitäten geeignet ist.
Mit diesem Ansatz konnten tausende Proteine quantifiziert werden, sodass eine umfangreiche
proteomische Datenbank von neuroendokrinen Tumoren allgemein und speziell des
Thymus und der Lunge geschaffen wurde. Außerdem zeigten sich zahlreiche signifikant
quantitativ deutlich unterschiedlich exprimierte Proteine in verschieden Vergleichen der
jeweiligen Tumorsubgruppen, welche als Biomarkerkandidaten zur teilweise schwierigen
diagnostischen Trennung zwischen neuroendokrinen Tumoren des Thymus und der Lunge
weiter untersucht werden können. Auffällig war hier eine starke Expression der Proteine
des MCM-Komplexes in den high grade Tumoren von Thymus und Lunge.
In der explorativen Datenanalyse zeigte sich, dass sich die Karzinoide (low und intermediate
grade Tumoren) beider Organe voneinander unterscheiden, während dies für die groß- und
kleinzelligen Karzinome nicht der Fall war. Weiterhin zeigte sich, dass sich die zuvor beschriebene
Subgruppe der TNET-G3 wie die Karzinoide verhält und sich klar von den
anderen großzelligen Karzinomen unterscheidet. Diese Umstände sollten für zukünftige
Einteilungen der neuroendokrinen Tumoren des Thymus berücksichtigt werden, damit die
betroffenen Patienten dem biologischen Verhalten der Tumoren nach diagnostisch korrekt
eingeteilt und entsprechend optimal behandelt werden können.
Die aktuelle WHO-Klassifikation, welche für die neuroendokrinen Lungentumoren erstellt
und für die Thymustumoren übernommen wurde, erscheint in dieser Hinsicht verbesserungswürdig.
Eine Diskussion um eine Erweiterung der bisherigen Kriterien, welche die
Zellmorphologie, Mitosezahl und das Auftreten von Nekrosen bewerten, um weitere Parameter
wie immunhistochemische Färbeergebnisse (Chromogranin, EZH2, Ki67-Index,
MCM-Proteine), erscheint unter Berücksichtigung der hier erzielten Ergebnisse angebracht
und nötig, insbesondere um die Subgruppe der TNET-G3 korrekt zu klassifizieren. | de |
dc.description.abstracteng | Neuroendocrine tumours of the thymus are very rare neoplasms. The closest related group of tumours are neuroendocrine tumours of the lung. Both are classified using the same criteria into four subtypes: typical carcinoid, atypical carcinoid, large cell neuroendocrine carcinoma and small cell (lung) carcinoma. Nonetheless there are also several differences between the two tumour groups, such as the frequency distribution of subgroups and genders and biological aggressiveness. Neuroendocrine tumours of the thymus show a worse prognosis, especially in the carcinoid subtypes.
In this project we performed a quantitative mass spectrometric proteomic analysis to increase the rather scarce knowledge about these rare tumours. Therefore we produced a specific protein quantification standard that can not only be used for neuroendocrine tumours of the thymus and lung but also for other neuroendocrine tumour entities as our data suggest.
With this approach we identified and quantified thousands of proteins and therefore created a proteomic databank of neuroendocrine tumours in general and for thymic and lung neuroendocrine tumours specifically. Moreover the analyses revealed numerous significantly quantitatively differentially expressed proteins in several comparions of the respective tumour subgroups, which can be further investigated as biomarker candidates for the occasionally difficult diagnostic separation of thymic and lung neuroendocrine tumours. There was a noticeable strong expression of proteins of the MCM complex in high grade tumours of the thymus and lung.
The explorative data analysis indicated a difference between the carcinoid tumours (low and intermediate grade tumours) of both organs, whereas this was not the case for the large and small cell carcinomas (high grade tumours). In addition, the data demonstrated that the beforementioned subgroup of TNET-G3 shows a very similar protein expression to the carcinoid tumours and is clearly differentiated from the other large cell carcinomas. These circumstances should be considered for future classification criteria of neuroendocrine tumours of the thymus, so that the affected patients are classified and treated correctly according to their biological behaviour.
The current WHO classification, which was created for lung neuroendocrine tumours and is used for the thymic tumours aswell, appears to be insufficient in this regard. In view of the results of this study, an extension of the existing criteria, which evaluate the cell morphology, mitotic count and the occurrence of necrosis, should be strongly considered. Additional parameters in this matter could be immunohistochemical expression of chromogranin, EZH2, Ki67 and MCM proteins with the aim to classify the TNET-G3 subgroup correctly. | de |
