The aim of the study was to analyze the effect of a therapy with strontium ranelate, teriparatide (PTH) and vertical whole-body vibration (VIB) of 70 Hz alone and in combination of strontium ranelate (SR) or PTH with VIB on skeletal muscle of ovariectomized rats. Three-month-old Sprague-Dawley rats were divided in seven groups of 10-11 animals each and except for the control group 1 (NON-OVX) ovariectomized. There were two control groups, one was not ovariectomized and not treated (control group 1; NON-OVX) and the other one was only ovarectomized (control group 2; OVX). Three groups were treated with VIB, PTH and SR in monotherapy. In two groups, combined therapies were applied which consisted of VIB and PTH or VIB and SR. Eight weeks after ovariectomy, a bilateral transverse osteotomy was performed at the tibia metaphysis in all rats to examine bone healing for another research study. PTH (40 µg/kg BW sc. 5x/week) and SR (613 mg/kg BW in food daily) were applied on the day of osteotomy for up to 42 days. Vibration treatments began 5 days after osteotomy (vertical, 70 Hz, 0.5 mm, twice a day for 15 min) and were conducted for up to 37 days. After the autopsy (14 weeks after ovariectomy), the gastrocnemius, the soleus and the longissimus dorsi muscles of all rats were extracted, weighed and histologically evaluated. In the muscles, capillary density, fiber cross-sectional areas and fiber diameter were analyzed. Further parameters studied were body weight, weight of the muscles und uterus weight. PTH alone affected fibers size less. The combination of PTH and VIB increased the capillary density in gastrocnemius muscle. The therapy with SR caused an increase in capillary density and decrease in fiber diameter and cross-sectional area (data relative to body weight) in gastrocnemius muscle. This effect can be explained due to the higher weight of the SR group. Meanwhile this medicament has been taken off the market. Therefore, it probably would not have any importance in the therapeutic usage anymore. Monotherapy with VIB had no influence on capillary density or fiber size. The results of a combined therapy of PTH and VIB indicate a positive synergistic effect related to the capillary density and fiber size and thus a positive effect on skeletal muscle. In the future VIB could play an important role in the prevention and therapy of sarcopenia because of its practical applicability. However, the treatment needs to be further studied regarding an optimal therapy regime with positive effect on both muscles and bone structures.

Ziel dieser Arbeit war es zu untersuchen, welchen Einfluss die Behandlungen – Strontiumranelat (SR), Teriparatide (PTH) und vertikale Ganzkörpervibration (VIB) mit 70 Hz einzeln und die Kombination aus PTH oder SR mit VIB - auf die Skelettmuskulatur von ovarektomierten Ratten haben. Dazu wurden drei Monate alte weibliche Sprague-Dawley Ratten in sieben Versuchsgruppen mit jeweils 10-11 Tieren eingeteilt und mit Ausnahme der Kontrollgruppe 1 ovarektomiert. Als Kontrollgruppen dienten zwei Versuchsgruppen, wobei die eine nicht ovarektomiert und nicht therapiert (Kontrollgruppe 1; NON-OVX) und die andere lediglich ovarektomiert (Kontrollgruppe 2; OVX) wurde. Drei Gruppen erhielten in einer Monotherapie PTH, SR oder VIB. Zwei Versuchsgruppen wurden einer Kombinationstherapie unterzogen, welche aus PTH und VIB oder SR und VIB bestand. Acht Wochen nach der Ovarektomie erfolgte eine bilaterale Osteotomie der Tibiae mit Osteosynthese für eine Paralleluntersuchung zum Thema Knochenheilung. Die medikamentöse Therapie begann ab dem Tag der Osteotomie und die Ganzkörpervibration fünf Tage später. Die Medikamente SR (Präparat Protelos in einer Dosis von 600-625 mg/kg KG/Tag im Futter) und PTH (Präparat Teriparatide in einer Dosis 40 μg/kg KG fünfmal pro Woche subkutan) wurden über einen Zeitraum von 42 Tagen verabreicht. Die VIB wurde erst 5 Tagen nach der Osteotomie über einen Zeitraum von 37 Tagen zweimal täglich zur selben Zeit über 15 Minuten bei einer Amplitude von 0,5 mm mit einer Frequenz von 70 Hertz durchgeführt. Die Untersuchung wurde am Ende der 14. Woche nach Ovarektomie durch Dekapitation beendet. Danach wurden Proben des M. gastrocnemius, M. longissimus dorsi und M. soleus entnommen, gewogen und histologisch untersucht. Die Analyse der histologischen Schnitte diente zur Bestimmung des Muskelfaserdurchmessers, des Muskelfaserquerschnittes sowie des Verhältnisses von Kapillaren zu Muskelfasern. Als weitere Parameter wurden die Körpergewichte, die Gewicht der einzelnen untersuchten Muskeln und die Uterusgewichte analysiert. PTH allein beeinflusste die Muskelfasergröße wenig. In Bezug auf die Kapillardichte ließ sich beim M. gastrocnemius durch Kombination von PTH und VIB ein positiver Effekt aufzeigen. SR führte beim M. gastrocnemius zu einer Steigerung der Anzahl der Kapillaren und zu einem geringeren Querschnitt und Durchmesser der Muskelfasern (Daten bezogen auf das Körpergewicht). Diese Effekte lassen sich jedoch durch das größere Körpergewicht in der mit SR therapierten Gruppe erklären. Dieses Arzneimittel wurde aber inzwischen vom Markt genommen und wird deshalb wahrscheinlich in der therapeutischen Anwendung keine Bedeutung mehr haben. VIB allein zeigte auf Kapillardichte und Muskelfasergröße keine Auswirkungen. Die Resultate der Kombinationstherapie von PTH und VIB könnten für einen positiv synergistischen Effekt in Bezug auf die Kapillardichte und die Muskelgröße sprechen und diese Therapie kann somit für die Muskulatur als positiv herausgestellt werden. Die Ganzkörpervibrationstherapie könnte aufgrund der einfachen Anwendbarkeit in Zukunft eine wichtige Rolle bei Prävention und Therapie der Sarkopenie spielen. Jedoch gilt es, in weiteren Studien ein optimales Therapieregime zu finden, dass sich sowohl für die Muskulatur als auch die Knochenstruktur als förderlich herausstellt.
