Due to the continuous development of medical treatment options, the question of the use of life-sustaining measures at the end of life is an important topic in everyday clinical practice. First it is shown an overview of the existing documentation options for end of life decisions. Subsequently, the sheet "Dokumentation einer Entscheidung über den Verzicht auf Wiederbelebung" from Göttingen is presented. In the framework of a two-part evaluation consisting of qualitative and quantitative research methods, the frequency and form of usage of the documentation sheet as well as the experiences with the document by the medical staff are being analyzed by using guided interviews. It is evident that the sheet, thanks to its clear structure, improves the reliability to the medical staff in dealing with the patient in an emergency situation and contributes to a medical discourse and the finding of a consensus within the team. The frequency of use could be increased by giving systematic instructions in handling the sheet, so that end of life decisions can be clearly noted. The mentioned barriers in communication with the team and the patient still remain as problems that cannot be solved by the documentation sheet. In the end, a satisfactory response to the question of the use of life-sustaining measures can only be achieved through clear, respectful and understanding communication between doctor and patient.

Angesichts der kontinuierlichen Weiterentwicklung medizinischer Behandlungsoptionen ist im klinischen Alltag die Frage nach dem Einsatz lebenserhaltener Maßnahmen am Ende des Lebens ein bedeutsames Thema. Zunächst wird ein Überblick über bestehende Dokumentationsmöglichkeiten von Entscheidungen am Lebensende gegeben. Anschließend wird die Göttinger "Dokumentation einer Entscheidung über den Verzicht auf Wiederbelebung" vorgestellt. Im Rahmen einer zweiteiligen, aus qualitativen und quantitativen Forschungsmethoden bestehenden Auswertung, werden zum einen die Nutzungshäufigkeiten und - formen des Dokumentationsbogens sowie zum anderen mittels leitfadengestützter Interviews Erfahrungen des medizinischen Personals mit dem Dokument analysiert. Hierbei fällt auf, dass der Bogen dem Personal durch seine klare Strukturierung für den Notfall mehr Sicherheit im Umgang mit dem Patienten verleiht sowie zu einem Diskurs und zur Findung eines Konsenses im Behandlungsteam beiträgt. Eine Steigerung der Nutzungshäufigkeit könnte durch eine systematische Einweisung in die Handhabung des Bogens geschehen, sodass im Team abgestimmte Entscheidungen nachvollziehbar vermerkt werden können. Die in den Interviews angesprochenen kommunikativen Barrieren im Team und mit dem Patienten stellen noch immer entscheidende Probleme im klinischen Alltag dar, die auch der Dokumentationsbogen nicht zu lösen vermag. Eine zufriedenstellende Beantwortung der Frage nach dem Verzicht auf lebenserhaltende Maßnahmen kann letztendlich nur durch eine offene, respektvolle und verständnisvolle Kommunikation zwischen Arzt und Patient erreicht werden.
