Neuropsychologische Testleistungen bei Patienten mit Koronarer Herzkrankheit und ihr Zusammenhang mit morphologischen Auffälligkeiten im cranialen Kernspintomogramm

dc.description.abstractKernspintomographische Untersuchungen zeigten bei Patienten mit Koronarer Herzkrankheit vermehrt lakunäre Läsionen und höhergradige Veränderungen der White Matter. Die negativen Auswirkungen der Koronaren Herzkrankheit auf die kognitive Leistungsfähigkeit der Patienten wurden in neuropsychologischen Studien ebenfalls nachgewiesen, so daß Patienten mit Koronarer Herzkrankheit als Risikogruppe für die Entwicklung einer vaskulären Demenz betrachtet werden. In jüngster Zeit zeigten neuropathologische Studien bei Patienten mit Koronarer Herzkrankheit auch Veränderungen wie senile Plaques und neurofibrilläre tangles im Temporallappen insbesondere im Hippocampus, die bisher als typisch für Patienten mit Alzheimer-Demenz galten. Die vorliegende Arbeit sollte bei Patienten mit Koronarer Herzkrankheit kernspintomographisch die Häufigkeit mikroangiopathischer Läsionen und die Volumina temporaler Strukturen erheben sowie den Zusammenhang dieser kernspintomographischen Befunde mit neuropsychologischen Testleistungen überprüfen. Siebenunddreissig Patienten wurden mit 24 gesunden Kontrollprobanden bezüglich kernspintomographisch identifizierbarer Auffälligkeiten wie Anzahl lakunärer Läsionen, Schwere der Veränderungen in der White Matter und Volumina temporaler Hirnstrukturen sowie neuropsychologischer Testleistungen verglichen. Es konnten deutliche Leistungsdefizite der Patienten im Bereich des expliziten und impliziten Gedächtnisses und des schlußfolgernden Denkens nachgewiesen werden. Die Gruppen unterschieden sich nicht in der Anzahl lakunärer Läsionen oder im Ausmaß der Veränderungen der White Matter. In beiden Gruppen waren höhergradige Veränderungender White Matter mit höherem Lebensalter und schlechteren kognitiven Testleistungen assoziiert. Im Vergleich mit den Kontrollprobanden zeigten Patienten mit Koronarer Herzkrankheit signifikant kleinere Hippocampusvolumina. Das Volumen des temporalen Pols und des temporalen Neocortex war nicht reduziert. Allerdings standen in der Patientengruppe kleinere Volumina des Hippocampus in einem Zusammenhang mit kleineren Volumina des temporalen Pols und des temporalen Neocortex sowie mit schlechteren neuropsychologischen Testleistungen. Kleinere Hippocampusvolumina von Patienten mit Koronarer Herzkrankheit und ihr Zusammenhang mit kleineren Volumina des temporalen Pols und des temporalen Neocortex können interpretiert werden als strukturelles Korrelat neurofibrillärer tangles und neuronalen Zelluntergangs im Temporallappen; neuropathologische Veränderungen, die bei Alzheimer-Patienten und in letzter Zeit auch bei Patienten mit Koronarer Herzkrankheit gefunden wurden.de

dc.description.abstractengRecent studies using structural MRI show more lacunar lesions and more white matter changes in patients with coronary artery disease. Cognitive decline has also been shown in these patients. Thus, patients with coronary artery disease may be considered a risk group for developing vascular dementia. More recently, neuropathologic studies have detected Alzheimer-related alterations like senile plaques and neurofibrillary tangles in medial temporal lobe structures, e.g. in the hippocampus of patients with coronary artery disease. The present study examined patients with coronary artery disease by structural MRI and cognitive tests in order to define the frequency of microangiopathic events and the volumes of temporal lobe structures. Thirty seven patients were compared to 24 matched controls with respect to the number of lacunar lesions, the extent of white matter changes, the volume of temporal lobe structures and cognitive functions. Patients with coronary artery disease showed deficits in explicit and implicit memory functions and executive functions like reasoning and concept formation. However, patients with coronary artery disease and control subjects did not differ in the number of lacunar lesions or the extent of white matter changes. In both groups, higher rates of white matter changes were related to older age and worse cognitive performance. Patients with coronary artery disease had significantly smaller left and right hippocampal volumes when compared with controls. Volumes of the temporal pole and the temporal neocortex were not reduced when compared with controls. However, smaller hippocampal volumes in patients with coronary artery disease were significantly related to smaller volumes of the temporal pole and the temporal neocortex and to worse cognitive performance. Smaller hippocampal volumes and the relationship to smaller volumes of the temporal pole and the temporal neocortex in patients with coronary artery disease may be interpreted as structural correlate of neurofibrillary tangles and neuronal death in the temporal lobes of patients with Alzheimer-dementia and, more recently, of patients with coronary artery disease.de
