dc.description.abstractIn der vorliegenden Arbeit wurde der Zusammenhang zwischen Patienten mit Major Depression nach DSM-IV und Schilddrüsenerkrankungen, unter besonderer Berücksichtigung einer Autoimmunthyreoiditis, untersucht.In einem Zeitraum von 14 Monaten wurden prospektiv 100 Patienten in die Studie eingeschlossen, von denen 50 Patienten über die Diagnose einer gesicherten Major Depression nach DSM-IV-Kriterien verfügten (Hauptgruppe) und 50 gesunde Blutspender die alters- und geschlechtsgematchte Kontrollgruppe bildeten.Von allen Patienten wurde eine kurze Anamnese erhoben, zudem wurde der Schilddrüsenstatus (TSHb, fT3, fT4, aTPO, aTG) untersucht. Bei den Patienten der Hauptgruppe wurde außerdem eine ausführliche Depressionsdiagnostik mittels M.I.N.I. nach DSM-IV, SDS (Self-Rating Depression Scale) und HAMD (Hamilton Depression Scale) durchgeführt zur standardisierten Feststellung und Quantifizierung vergangener und aktueller Depressionen.Ein möglicher Zusammenhang zwischen Depressionen und einem vermehrten Auftreten einer Autoimmunthyreoiditis konnte in dieser Arbeit nicht nachgewiesen werden, müsste aber an größeren Kollektiven weiter untersucht werden. Auffällig und statistisch signifikant war allerdings, dass bei Patienten mit Depressionen im Vergleich zur Kontrollgruppe fast doppelt so häufig pathologische Schilddrüsenwerte auffielen. Zudem scheint es bei den Patienten der Hauptgruppe einen Trend zu häufigeren Über- und Unterfunktionen der Schilddrüse zu geben als in der Kontrollgruppe. Ebenso konnte bezüglich des Alters bei älteren Patienten (40-79 Jahre) der Hauptgruppe eine statistisch signifikante pathologische Erhöhung der Schilddrüsenparameter beobachtet werden.de

dc.description.abstractengIn the present study we tried to figure out a relationship between Major Depression (after DSM-IV criteria) and thyreoid diseases, especially autoimmune thyreoiditis.For fourteen month we examined 100 patients: 50 with the diagnosis Major Depression (main group) and 50 healthy blood donors (control group) from the department of transfusion medicine of the Georg-August-University Göttingen.All patients underwent a complete thyroid evaluation including thyroid-stimulating hormone (TSH), free T3 (fT3), free T4 (fT4), antithyroid peroxidase antibodies (aTPO) and thyreoglobulin antibodies (aTG).The presence of psychiatric disorders in the main group was assessed by means of the M.I.N.I. (Mini International Neuropsychiatric Interview, German version 5.0.0), SDS (Self-Rating Depression Scale) and HAMD (Hamilton Depression Scale).In this study we found no association between depression and antithyroid antibodies. It s quite possible, that the small study-population is a reason for this result.To sum it up, we observed, that patients with depression showed nearly twice the number of pathological thyroid hormones compared with the control group. The study seems to suggest that patients of the main group had more often hyperthyroidism or hypothyroidism than in the control group. As well we found in the older main group (aged 40-79 years) a statistical significant increase of blood thyroid hormones.de
