dc.description.abstractHintergrund: Zerebrale mikroembolische Läsionen sind die häufigsten Komplikationen der Stentgeschützten Perkutanen Angioplastie der Karotiden (SPAC). Diese gelten als Folge thrombembolischer Abscheidungen aus der Karotisplaque während der Intervention. Plaquezusammensetzung und morphologie spielen eine Rolle bei der Stabilität der Plaque und könnten damit einen prognostischen Marker für das Auftreten von Mikroembolien während der Intervention darstellen. Bisher bleibt unklar inwieweit zerebrale Protektionssysteme diese Läsionen verhindern können. Methode: 48 Fälle einer symptomatischen und/oder hochgradigen Karotisstenose wurden in die prospektive Studie eingeschlossen und einer SPAC zugeführt. Präinterventionell wurden die Karotiden mittels CT-Angiographie dargestellt und nach morphologischen Kriterien wie Plaquezusammensetzung analysiert. Innerhalb von 48 Stunden jeweils vor und nach Intervention wurde ein zerebrales DWI-MRT erstellt und auf mikroembolische Infarkte untersucht. Ergebnisse: Der Weichplaqueanteil korrelierte positiv mit der Zahl neuer DWI-Läsionen nach SPAC (p=0.09) und signifikant mit der Fläche neuer DWI-Läsionen (p=0.05). Ulzerierte Plaques gingen mit mehr neuen DWI-Läsionen einher als nicht-ulzerierte. Andere morphologische Kriterien korrelierten mit der Läsionslast. Schlussfolgerung: Das Verhältnis von Weichplaque zu Hartplaque, Plaqueulzerationen und-morphologie haben einen Einfluss auf das Auftreten von mikroembolischen Ereignissen während der SPAC.de

dc.description.abstractengBackground: Microembolic lesions are the most frequent complications of stent-protected angioplasty of the carotids (SPAC). They are thought to be thrombembolisms of the carotid plaque. Plaque morphology and composition have influence on cerebral microembolic events during SPAC and could serve as predictive markers for microembolic events. Although cerebral protection devices are used to reduce this complication, they cannot entirely prevent from it. Methods: We included 48 cases of symptomatic and/or high-grade carotid stenosis in our prospective study. All cases underwent SPAC und prior to this CT-angiography to analyze plaque composition and morphology. Within 48 hours before and after intervention we performed cerebral DWI-MRI to analyze microembolic events during SPAC. Results: Proportion of soft-plaque correlated with the quantity of new DWI-lesions after intervention (p=0.09) and we found significant correlation with the area of new DWI-lesions (p=0.05). Ulcerated plaques were associated with more new DWI-lesions than non-ulcerated plaques. Other criteria of plaque morphology also correlated with the quantity of new DWI-lesions. Conclusion: The proportion of soft-plaque, plaque ulceration and plaque morphology have influence on cerebral microembolic events during SPAC.de
