dc.description.abstractDa Frühgeborene ein deutlich erhöhtes Risiko für Hör- und Sprachstörungen haben, sollten in einem Follow-up peripheres Hören und Sprachentwicklung frühgeborener Kinder zu den FAEP ihrer postnatalen BERA in Beziehung gesetzt werden. Stichprobe und Methode: Neunundreißig Frühgeborene mit einem bestimmten Risikoprofil wurden postnatal (Min 33., Max 152. Lebenstag) einer BERA unterzogen; alle waren hörgesund (Hörschwelle beidseits 20 dB). Im 3. Lebensjahr wurden Ohranamnese und Ohrinspektion mit mikroskopie durchgeführt, OAE abgeleitet, anamnestische Angaben der Eltern zur Entwicklung erhoben und der aktuelle Sprachentwicklungsstand mit einem standardisierten Test (SETK-2) ermittelt. Der Zusammenhang zwischen der Interpeaklatenz der Wellen I V (IPL in ms als Maß der neuronalen Reifung) und somatischen wie auch Sprachentwicklungsparametern wurde berechnet. Ergebnisse: Es bestand kein statistisch signifikanter Zusammenhang von IPL und Geburtsgewicht bzw. Körperlänge bei Geburt, jedoch von IPL und den Gestationsaltern bei Geburt (- 0,43; p=0,02) respektive zum BERA-Zeitpunkt (-0,61; p=0,0003). Je niedriger diese Alter waren, desto länger war die IPL. Zwischen IPL und Lebensalter bei Sprechbeginn (erste Worte; 2-Wort-Satz) bestand ebenso wenig ein Zusammenhang wie mit den gemessenen Sprachtestleistungen. Fazit: Die IPL hing als Maß der neuronalen Reifung vom Gestationsalter ab, korrelierte jedoch nicht mit der Sprachentwicklung im 3. Lebensjahr. Rezidivierende Otitiden und Paukenergüsse ließen sich als risikohaft für die Sprachentwicklung evaluieren.de

dc.description.abstractengBackground: Preterm born children are at increased risk of hearing- and language impairment. The present study aimed to investigate the association of FAEP of postnatal BERA with peripheral hearing and language development of premature infants in a follow-up setting. Methods: Thirty-nine preterm children with a defined risk profile were examined by BERA postnatally (min 33. and max 152. day of life). During the third year of life an inspection and microscopical examination of the ear were performed as well as otoacoustic emission tests, data on the development history were taken by the parents and the state of language development was assessed by means of a standardised test (SETK-2). The waves interpeak latency I V (IPL in ms as parameter of the neuronal maturation) was correlated with somatic parameters and language developmental parameters. Results: There was no statistically significant correlation between IPL and weight or height at the time of birth, but between IPL and the gestational age at the time of birth (-0,43; p=0,02) and gestational age at the time of BERA, respectively (-0,61; p=0,0003). The younger the gestational age, the longer the IPL. There was neither an association between IPL and the age of beginning of speaking (first words; 2- word-sentences) nor with the results of the language development test. Conclusion: The IPL, a parameter of the neuronal maturation, was dependent on the gestational age, but did not correlate with the language development in the third year of life. Recurrent otitis media and middle ear effusion were found to be risk factors for impaired language development.de
