dc.description.abstractZiel der Studie war es, einen Überblick über das Mundhygieneverhalten und die orale Gesundheit bei Patienten vor und nach Organtransplantation zu gewinnen und den Kenntnisstand der Patienten über die Korrelation von Mundgesundheit und Transplantation zu eruieren. Die Untersuchung gliederte sich in zwei Teile. An die allgemeine und zahnärztliche Anamnese zur Mundgesundheit anhand von zwei Fragebögen schloss sich die intraorale Inspektion der Patienten an. Diese umfasste einen eingehenden Befund zur Erhebung des DMF-T (Decayed Missing Filled Teeth) sowie die Aufnahme von Parodontalem Screening Index (PSI) und QHI. Dieser erfasst den Plaquebefall der Zahnflächen graduell. Vorhandene Panoramaschicht-aufnahmen (PSA) wurden nach der direkten Millimetermethode auf parodontale Destruktion hin untersucht. Die statistische Auswertung erfolgte mit dem Mann-Whitney-U-Test, dem t-Test und dem Χ2-Test. Das Signifikanzniveau lag bei p<0,05. Es konnten 20 Patienten vor (Alter 52,6±13,5! ) und 20 Patienten nach Herz-, Leber- oder Nierentransplantation in die Studie einbezogen werden (Alter 53,9±13,8). Zudem war bei vier Patienten einer ähnlichen Studie aus dem Jahr 1996/97 eine Nachuntersuchung möglich. Aufgrund der geringen Teilnehmerzahl wurden diese Ergebnisse nicht mit in die Statistik einbezogen. Die Auswertung der Fragebögen zeigte keine signifikanten Unterschiede hinsichtlich der Mundhygiene und des Informationsgrades der Patienten. In beiden Gruppen waren 70% nicht über die Korrelation von oraler Gesundheit und Transplantation aufgeklärt worden. Statistisch relevant war das Interesse der Patienten an einer eingehenden Mundhy-gieneunterweisung (p=0,017). 35% der transplantierten Patienten wünschten keine Informationen zur richtigen oralen Hygiene. Die mittleren DMF-T-Indizes der Gruppe vor (18,1±5,28) und der Gruppe nach Transplantation (16,7±6,5) zeigten ebenso wie die Sanierungsgrade vor (75,09%±27,81) und nach Transplantation (78,27%±31,39) keine signifikanten Unterschiede. Gesunde parodontale Verhältnisse (PSI Code 0) ergaben sich bei keinem der Studienteilnehmer. Beim QHI hingegen ergaben sich für die Gruppe nach Transplantation mit 2,43±0,723 signifikant höhere Werte als für die Patienten vor Transplantation (1,89±0,764). Beim radiologisch bestimmten Attache-mentverlust mesial und distal konnten keine Unterschiede ermittelt werden. Die Ergebnisse der Studie zeigen deutliche Defizite der zahnärztlichen Betreuung von Patienten vor und nach Transplantation. Die signifikant schlechtere Mundhygiene der Gruppe nach Transplantation könnte durch die immunsuppressive Therapie sowie den im Vergleich zur Transplantationsnachsorge reduzierten Stellenwert der oralen Gesundheit bedingt sein. Eine Aufklärung der Patienten über den Zusammenhang von Mundgesundheit und Transplantation sollte sowohl von ärztlicher als auch zahnärztlicher Seite erfolgen.de

dc.description.abstractengThe study aims to describe the oral hygiene of patients before and after solid organ transplantation and to evaluate their knowledge about the correlation between oral health and organ transplantation. The exploration consisted of two parts: at first all patients were asked to answer two questionnaires about their medical history and aspects of their oral hygiene. This was followed by an intraoral examination of all patients. This examination consisted of the DMF-T (Decayed Missing Filled Teeth), the PSI (Periodontal Screening Index) and the QHI describing the plaque accumulation. Existing orthopantomograms were examined for periodontal destruction using the direct millimeter method. Statistical analysis were performed with the Mann-Whitney-U-Test, the t-Test and the Χ2-Test. The level of significance was set at p<0.05. 20 patients before (age 52.6±13.5) and after heart, liver or kidney transplantation (age 53.9±13.8) participated in the study. Four additional patients of a similarly study from 1996/97 were examined. Because of the small turnout the results of these group were not statistically analysed. The questionnaires showed no differences between the two groups. Oral hygiene and knowledge of oral health were compareable. In both groups 70% of the patients were not informed about the correlation between organ transplantation and the importance of oral health. Statistically relevant was the patients` interest in a detailed oral hygiene briefing (p=0.017). 35% of the transplanted patients did not want to receive further information about oral health. The DMF-T-index of the patients before (18.1±5.28) and after (16.7±6.5) transplantation showed no differences. Periodontal health (PSI code 0) was found in none of the examined patients. In contrast to these findings the QHI showed statistically higher plaque rates for the transplanted patients (2.43±0.723 vs. 1.89±0.764). The radiographic attachement loss mesial and distal resulted in no differences. The study shows a lack of dental assistance for patients before and after organ transplantation. Information of the correlation between oral health and transplantation should be provided from both dentists and physicians.de
