This retrospective analysis describes the use of additive intracoronary thrombolysis with recombinant tissue plasminogen activator in comparison to percutaneous coronary intervention alone in treatment of stenotic or occluded coronary artery bypass grafts. Hospital charts and coronary angiography films of 127 patients with disfunction of coronary artery bypass grafts were reviewed. 32 patients were treated with intracoronary infused recombinant tissue plasminogen activator in addition to percutaneous coronary intervention, 95 patients were treated only with conventional percutaneous coronary intervention. Despite combination of anticoagulant and thrombolytic therapy there was neither bleeding requiring transfusion nor a cerebral bleeding recorded in patients with local recombinant tissue plasminogen activator treatment. Percutaneous coronary intervention of coronary artery bypass grafts in addition to intracoronary recombinant tissue plasminogen activator treatment seems to ! be an effective and safe alternative method in therapy of coronary artery bypass graft dysfunction.

Die vorliegende Arbeit untersucht die Auswirkungen einer adjuvanten lokoregionalen Thrombolyse mit recombinant tissue plasminogen activator in der Therapie stenosierter und verschlossener aortokoronarer Venenbypässe. Dieser Untersuchung wurde eine systematische retrospektive Auswertung der Therapieverläufe und Koronarangiographiefilme von insgesamt 127 Patienten mit einer Bypassdysfunktion zugrunde gelegt. 32 Patienten wurden zusätzlich zur Bypass-PCI mit recombinant tissue plasminogen activator behandelt, bei den übrigen 95 Patienten wurde eine konventionelle Bypass-PCI vorgenommen. Trotz der kombinierten antikoagulatorischen und thrombolytischen Therapie wurden in der Gruppe LYSE außer lokalen Blutungen, insbesondere Leistenhämatomen, keine lebensbedrohlichen transfusionspflichtigen oder zerebralen Blutungskomplikationen beobachtet. Die vorliegenden Daten belegen, dass eine perkutane koronare Venenbypassintervention mit gleichzeitiger lokoregionaler Thrombolyse mit rt-PA eine wirksame und sichere Therapiealternative für stenosierte und verschlossene degenerierte aortokoronare Venenbypässe darstellt.
