dc.description.abstractEinleitung: In neueren biomechanischen Studien zeigte die double-row-Naht-Anker-Rekonstruktion zur operativen Therapie der Rotatorenmanschettenruptur gegenüber der single-row-Technik bessere biomechanische Resultate. Das Ziel dieser Studie war die histologische und immunhistologische Untersuchung der Kollagenverteilung, während des Heilungsprozesses der Knochen-Sehnen-Kontaktstelle nach single- und double-row-Nahtankerrekonstruktion. Material und Methoden: Diese Studie untersuchte die Sehnen-Knochenheilung am Schafmodell. Bei 22 reifen weiblichen Merino-Schafen wurde eine standardisiert erzeugte Ruptur der Infraspinatussehne refixiert. Zwei unterschiedliche Techniken wurden für die Refixation angewandt. Bei der single-row-Technik kamen zwei doppelt geladene Nahtanker, kombiniert mit arthroskopischer Mason-Allen-Naht, zum Einsatz. Bei der double-row-Refixation kamen vier doppelt geladene Nahtanker in Kombination mit arthroskopischer Mason-Allen-Naht zur Anwendung. Die Lokalisation der Kollagene Typ I, II und III während der Sehnen-Knochen-Heilung wurde 1, 2, 3, 6, 12 und 26 Wochen nach Chirurgie durch den Einsatz von konventioneller Histologie und Immunhistologie ausgewertet. Ergebnisse: Der Gewebeumbau ist sechs Wochen nach double-row-Rekonstruktion fast abgeschlossen, da kein positiver Kollagen-III-Nachweis erkennbar war, während dieser Prozess in der single-row-Gruppe noch bis zur 12. Woche andauert. Die immunhistologischen Ergebnisse zeigten eine Verschiebung der Kollagen-Ratio. Ab der 12. Woche fand in beiden behandelten Gruppen eine Erhöhung von Kollagen Typ I in der Sehne und im Knochen statt. Diskussion: Die arthroskopische Rotatorenmanschettenrekonstruktion ist mittlerweile eine häufig angewandte und beschriebene Operationstechnik in der Schulterchirurgie. Die meisten biomechanischen Untersuchungen zeigten für double-row-Rekonstruktion eine signifikant höhere Belastbarkeit, eine kleinere Elongation unter zyklischer Belastung und bessere anatomische Wiederherstellung des Sehnenansatzes am Knochen im Vergleich zu single-row-Techniken (Cummins et al. 2005, Ma et al. 2004,2006, Kim et al. 2006, Meier und Meier 2006, Milano et al. 2008). Diese Studie zeigt, dass die double-row-Technik im Vergleich zur single-row-Technik zu einem zellbiologisch günstigeren Sehnen-Knochen-Heilungsprozess führt.de

dc.description.abstractengIntroduction: In recent biomechanical studies, the double-row suture anchor reconstruction for surgical treatment of rotator cuff tears showed better biomechanical results when compared to the single-row technique. The aim of this study was the histological and immunohistochemical determination of the collagen distribution during healing of the bone-tendon contact point after single-and double-row suture anchor reconstruction. Materials and methods: This study examined the tendon-bone healing in a sheep model. In 22 mature female merino sheeps a standardized produced tear of the infraspinatus tendon has been reattached. Two different techniques were used for the reattachment. In the single-row technique, two double-loaded suture anchors combined with arthroscopic Mason-Allen suture were applied. In the double-row suture anchor refixation four double-loaded suture anchors in combination with arthroscopic Mason-Allen suture were carried out. The localization of collagens type I, II and III during the tendon-bone healing was evaluated at 1, 2, 3, 6, 12 and 26 weeks after surgery by the use of conventional histology and immunohistochemistry. Results: Six weeks after the double-row reconstruction tissue remodeling is nearly complete, there was no collagen III to be identified. However, in the single-row-group this was only seen at week 12. The immunohistochemical results showed a shift in the collagen ratio. From week 12 onwards, in both treated groups an increase of type I collagen occurred in the tendon and the bone. Discussion: Arthroscopic rotator cuff reconstruction is now a frequently used surgical technique in shoulder surgery. Most biomechanical studies have shown a significantly higher power, a smaller elongation under cyclic loading and better anatomical restoration of the tendon to the bone for double-row reconstructions as compared to single-row techniques (Cummins et al. 2005, Ma et al. 2004 , 2006, Kim et al. In 2006, Meier and Meier, 2006, Milano et al. 2008). This study demonstrated that the double-row technique, in comparison to single-row technique, leads to a cell biologically better tendon to bone healing process.de
