dc.description.abstract | Das Studienziel war, Alternativen für die körperliche Nachuntersuchung von Hüfttotalendoprothesen zu finden, wenn nur eine Fernabfrage möglich ist. Es wurde evaluiert, welche Patientencharakteristika und Fragen aus Fragebögen signifikante Unterschiede zwischen Patienten mit und Patienten ohne radiologische Lockerungszeichen ermitteln können. Einer der Fragebögen war der modifizierte Nottingham Health Profile . Die Lockerung wurde im Röntgenbild definiert. Die Patienten der Lockerungsgruppe waren signifikant älter. Es gab deutlich mehr Frauen in der Lockerungsgruppe. Hinsichtlich Größe, Gewicht und BMI gab es keine signifikanten Unterschiede. Die Lockerungspatienten gaben häufiger an, ihre Prothese sei nur schlecht benutzbar, in vielen Dingen eingeschränkt zu sein und nur liegen und sitzen zu können. Sie hatten häufiger Schmerzen und nahmen Analgetika ein. In keinem Fall war der Unterschied signifikant, jedoch suggerierte die Antwortverteilung eine erkennbare Abweichung. Die fehlende Signifikanz kann statistischer Natur sein und ist möglicherweise durch die geringe Größe der Lockerungsgruppe bedingt. Der Nottingham Health Profile untersuchte sechs Problembereiche: Energieverlust , Schmerz , Physische Mobilität , Emotionale Reaktion , Schlafprobleme und Soziale Isolation . Weder in den einzelnen Bereichen noch in der Gesamtbetrachtung unterschied sich die Lockerungsgruppe signifikant von der anderen Gruppe. Wir schlussfolgerten, dass durch das Nottingham Health Profile eine sensitive Erfassung von Patienten mit radiologischen Lockerungszeichen im Frühstadium nicht möglich ist. Jedoch ist seine Verwendung hilfreich bei der Erkennung von klinisch manifesten Lockerungen. Das klinische Bild lässt sich mit dem Nottingham Health Profile grob abschätzen. Die Röntgenbildanalyse bleibt weiterhin ein unverzichtbarer Bestandteil der Diagnostik in der Nachsorge von Hüfttotalendoprothesen. Die geringe Größe unseres Kollektivs, insbesondere unserer Lockerungsgruppe, erlaubt es uns nicht, endgültige Schlüsse zu ziehen. Hierzu müssten weitaus größere, vorzugsweise nationale, prospektive Studien angelegt werden. Solche Studien würden durch die Einführung eines nationalen Endoprothesenregisters leichter ermöglicht werden. | de |

dc.description.abstracteng | The aim of the study was to find remote query alternatives for the physical examination of total hip replacement. Significant distinction in patient characteristics and questions from questionnaires was evaluated to differentiate between patients with and patients without radiological loosening. One of the questionnaires was the modified Nottingham Health Profile . Endoprosthetic loosening was defined in radiographs. The patients with loosening signs were significantly older. There were more women in the loosening group. There were no significant differences concerning height, weight and BMI. Patients with prosthetic loosening more often claimed that their implant was dysfunctional, to be limited in their activities and only to be able to lie and sit. They complained more often about pain and used analgetics. In none of the cases was the difference significant but the dispersion suggested a noticeable drift. Because of the small size of the loosening group the lacking significance may be due to statistical reasons. The Nottingham Health Profile analysed six sections: Energy , Pain , Mobility , Emotional reaction , Sleep and Social isolation . Neither for a particular section nor for the overall view was there a significant difference between the loosening group and the other group. We concluded t hat the Nottingham Health Profile is not able to identify patients with early prosthetic loosening. However, it is a helpful device to detect clinically manifest prosthetic loosening. The Nottingham Health Profile can give a rough estimate of the clinical condition of the patient. The conventional radiograph remains an essential part in the diagnostic aftercare of total hip replacement. The small size of our collective, especially of the loosening group, does not allow us to draw final conclusions. For this purpose it is necessary to set up larger, preferably national prospective studies. Such studies would be facilitated by launching a national arthroplasty register. | de |
