M: The aim of the study was to compare four different intracanal irrigation systems in their effect on the removal of a root canal sealer. METHODOLOGY: The root canals of fifty-three single-rooted extracted human teeth were instrumented and the roots split in longitudinal direction. In each tooth half artificial lateral grooves were prepared. The root canal surface and the artificial lateral grooves were covered with the sealer AH Plus. The teeth were divided into four experimental groups (n= 12) and one control group (n= 5). Compared were the effects of RinsEndo, ultrasonic irrigation, EndoActivator and CanalBrush. The control group was manual irrigation. All five groups were irrigated with sodium hypochlorite(1%/3%). The evaluation of remaining sealer was performed under a microscope with 30x magnification. RESULTS: Only group 2 (ultrasonics) showed significantly less remaining sealer compared to all other groups. Group 3 (EndoActivator) showed minimally superior cleaning effect in the coronal third of the canal. Group 4 (CanalBrush) showed no cleaning effect at all but demonstrated smearing of the sealer as well as retention of instrument components inside the root canal. None of the 5 groups had a statistically significant effect on cleaning the lateral grooves. Only in group 2 (ultrasonics) a slight but statistically non-significant effect on the lateral grooves could be observed. CONCLUSIONS: Within the limitations of this study protocol ultrasonic irrigation shows a superior effect on removing sealer from the root canal surface during endodontic retreatment. Cleaning of the lateral grooves seems not to be possible at all with one of the techniques investigated.de
