dc.description.abstract | Anaplastische Astrozytome sind aufgrund ihres diffus infiltrierenden Wachstums trotz intensiver Forschung noch heute mit einer schlechten Prognose assoziiert. In den letzten Jahren wird immer deutlicher, dass über das histologische Grading hinaus molekulargenetische Profile bessere Möglichkeiten bieten, Progressions- und Überlebensverhalten von Tumoren einzuschätzen. Mittels komparativer genomischer Hybridisierung (CGH) konnte in dieser Untersuchung gezeigt werden, dass bei primären anaplastischen Astrozytomen typischerweise chromosomale Aberrationen in Form von Zugewinnen an 7q sowie Verlusten an Chromosom 10 auftreten. Diese Veränderungen gehen mit einer schlechten Prognose einher. Verluste an 9p traten zunehmend als früher Progressionsmarker sowohl in rezidivierten anaplastischen Astrozytomen sowie in sekundären Glioblastomen auf. Eine entscheidende Rolle spielen zudem Aberrationen an 10p. Während Zugewinne einen stabilisierenden Effekt auf die Histologie im Rezidiv haben und sich günstig auf das Überleben auswirken, verhält es sich bei Verlusten gegensätzlich. Auch 13q-Verluste wirkten sich als Sekundäraberration prognostisch ungünstig aus. Die Untersuchungsergebnisse führten zur Erstellung eines Tumorprogressionsmodells anaplastischer Astrozytome, deren prognostische und diagnostische Marker bisher keinen oder nur geringen Eingang in Routinediagnostik und klinische Protokolle gefunden . Weitere Analysen genetischer und epigenetischer Pathologien sowie die Identifizierung molekulargenetischer Subtypen werden helfen, in der Zukunft noch bessere Aussagen bezüglich Diagnose, Gradierung und Klassifikation anaplastischer Astrozytome zu treffen. | de |

dc.description.abstracteng | Despite intensive investigation anaplastic astrocytoma are still linked with poor prognosis because of their diffuse infiltrating growth. In recent years it has become clear that molecular profiles beyond histological grading offer better opportunities to evaluate tumor progression and survival. This study used comparative genomic hybridization (CGH) to show that typical chromosomal aberrations in primary anaplastic astrocytoma appear as 7q-gains and losses on chromosome 10. These aberrations are associated with poor survival. Frequent losses on 9p are markers of the progression in recurrent anaplastic astrocytoma as well as secondary glioblastoma. Aberrations on 10p play a decisive role. While gains show an histology-stabilizing effect and better survival in recurrent anaplastic astrocytoma, losses implicate an adverse effect. Furthermore, losses on 13q go along with a poorer prognosis. The results of this study helped to develop a scheme of anaplastic astrocytoma progression. The discovered prognostic and diagnostic markers have not yet entered routine diagnostics or clinical protocols. Further studies of genetic and epigenetic pathologies as well as identification of moleculargenetic subtypes may help to improve diagnostics, grading and classification of anaplastic astrocytoma. | de |
