dc.description.abstractIn dieser Dissertation wird ein statistisches Verfahren zur Planung und Auswertung von Nichtunterlegenheitsstudien mit unabhängigen rechtszensierten Ereigniszeiten im Gold Standard Design entwickelt. Bei Studien im Gold Standard Design werden Patienten auf drei Therapiegruppen randomisiert verteilt, in denen sie entweder mit dem zu testenden Medikament (Novum), einem Standardmedikament (aktive Kontrolle) oder einem Placebo behandelt werden. Ziel solch einer Nichtunterlegenheitsstudie ist im Wesentlichen der Nachweis, dass der durch das Standardmedikament erzielte Therapieeffekt im Vergleich zu Placebo zu einem bestimmten Teil auch durch das Novum erzielt werden kann und somit das neue dem Standardmedikament nicht klinisch unterlegen ist. Wir entwickeln einen semiparametrischen Ansatz im Rahmen des Cox-Proportional-Hazards-Modells, bei dem lediglich angenommen wird, dass die Hazardfunktionen proportional zueinander sind. Speziell wird ein Algorithmus vorgestellt, mit dem die minimal benötigte Anzahl an Patienten und deren optimale Aufteilung in die Behandlungsgruppen bestimmt werden kann, so dass für eine bestimmte Parameterkonstellation in der Alternative eine bestimmte statistische Güte erreicht wird. Für diese Berechnung werden die Endpunkte als Weibull-verteilt angenommen. Mit Hilfe von Simulationsstudien wird der tatsächliche Fehler 1. Art sowie Güteeigenschaften des Testverfahrens und die berechneten Stichprobenumfänge untersucht. Schließlich vergleichen wir unsere Methode mit einer bereits existierenden, die auf exponentialverteilten Endpunkten basiert. Um unser Verfahren zu veranschaulichen wird eine randomisierte, aktiv- und placebokontrollierte, doppelblinde Studie bei schwerer Depression ausgewertet.de

dc.description.abstractengIn this dissertation we develop a statistical method for planning and evaluating non-inferiority trials for right censored time to event data in the gold standard design. In clinical trials in the gold standard design, patients are randomized into three treatment groups: either they are treated by the test treatment or the reference one (also called active control) or by a placebo. In general, the objective is to demonstrate that the new treatment is clinically not inferior to the active control where both are compared to placebo. We propose a semiparametric approach which only assumes the proportionality of hazard functions in the Cox model framework. In particular, we develop an algorithm for calculating the minimal total sample size and its optimal allocation to treatment groups such that a desired power can be attained for a specific parameter constellation in the alternative. For this calculation the endpoints are assumed to be Weibull distributed. By means of simulation studies, the actual type I error rate, power properties and the accuracy of the calculated sample sizes are investigated. Finally, we compare our procedure with an existing one where the endpoints are assumed to be exponentially distributed. To illustrate our method, a randomized, active and placebo controlled and double-blind trial in major depression is evaluated.de
