dc.description.abstract | Kolorektale Karzinome stellen die
zweithäufigste Krebserkrankung in Deutschland dar. Sowohl spontan
aufgetretene als auch ererbte genomische Veränderungen können die
Entstehung eines kolorektalen Karzinoms begünstigen. Genomische
Analysen auf MSI gelten heute als Standard in der Diagnostik
kolorektaler Karzinome. Das Patientenkollektiv dieser Arbeit
rekrutiert sich aus der CAO/ARO/AIO-04-Studie, sowie aus ihrer
Vorläuferstudie CAO/ARO/AIO-94. Untersucht wurde das
Patientenkollektiv auf MSI. MSI-Analysen werden als Biomarker und
Prädiktor für das Überleben eines Patienten und das Ansprechen auf
die Therapie herangezogen. MSI gilt heute als günstiger
Prognosefaktor für das kolorektale Karzinom. Die Funktion der MSI
als Prädiktor für eine Chemoresistenz gegen 5-FU wird derzeit
diskutiert. Untersucht wurde die DNA von 65 Patienten mit
Rektumkarzinomen vor und nach neoadjuvanter Chemoradiotherapie auf
MSI. Parallel dazu wurden die Gewebeproben immunhistochemisch
ausgewertet durch Mitarbeiter des Teilprojektes 5 der KFO 179. Als
Methoden wurden Mikrodissektion, PCR und Fragmentlängenanalyse
gewählt. Die Ergebnisse wurden bezüglich zweier Fragestellungen
ausgewertet: Hat die neoadjuvante Chemoradiotherapie einen Effekt
auf den MSI-Status? Und: Ist der MSI-Status als Biomarker und
Prädiktor für das Überleben und das Ansprechen auf die Therapie
geeignet? Von 65 prätherapeutisch untersuchten Proben zeigte sich
in einem Fall MSI-Low. Von 7 immunhistochemisch auffälligen
posttherapeutischen Proben zeigten 2 einen neu aufgetretenen
MSI-Low-Phänotyp. In der Immunhistochemie wurde parallel eine
Veränderung des Ausfalls der Mismatch-Repair-Gen-Expression
zwischen Tumoroberfläche und der Invasionstiefe sichtbar. An der
tiefen Invasionsfront des Tumors nahm die Expression ab. Folgende
Thesen lassen sich aus den vorliegenden Ergebnissen ableiten: 1.
MSI-High ist in Rektumkarzinomen selten zu beobachten. 2.
Rektumkarzinome zeigen eine erhöhte MSI-Low-Frequenz nach
neoadjuvanter Chemoradiotherapie. Der MSI-Status ist durch
Chemoradiotherapie beeinflussbar. Eine Eignung von MSI als
Prädiktor für eine mögliche Chemoresistenz gegen 5-FU zeichnet sich
aus den vorliegenden Ergebnissen der CAO/ARO/AIO-04-Studie ab. Es
ergibt sich somit eine neue Hinweisquelle für Prognose und Verlauf
der Erkrankung. | de |

common cancers in Germany. Spontaneously occurred gene damages as
well as hereditary acquired gene damages may cause cancer. Analyses
for MSI are performed as standard in the diagnosis system of
colorectal carcinomas. The patient population is based on two
studies: CAO/ARO/AIO-04 trial and its precursor trial
CAO/ARO/AIO-94. The patient population was examined for MSI.
Analyses for MSI are taken as biomarker and predictor for patients'
survival and their response to treatment. MSI is known as a
positive prognostic factor for colorectal carcinomas. The role of
MSI as a predictor for chemoresistance against 5-FU is currently
under discussion. DNA of 65 patients with rectal cancer was
analysed for MSI previous to therapy and after chemo-radiotherapy.
In addition other associates of sub-project 5 of the KFO 179
evaluated the tissue samples immunohistochemically. The methods
were microdissection, PCR and DNA fragment analysis. Results were
analyzed regarding two issues: Does chemo-radiotherapy produce an
effect on MSI state? Furthermore: Is the MSI state qualified to be
used as a biomarker and predictor for patients' survival and their
response to treatment? Among 65 tissue samples which were analysed
previous to therapy, one tissue sample showed MSI-low. Among 7
tissue samples which had exhibited immunochistochemical changes
after chemo-radiotherapy, two tissue samples showed a new
MSI-low-phenotype. At immunohistochemistry differences were
observed between the superficial mismatch-repair-gene-expression
and the expression at the deep invasion front of the tumor. The
expression decreased at the deep invasion front. Two theses may be
deduced from the results presented: 1. MSI-high is rare in rectal
carcinomas. 2. The frequency of MSI-low increases after neoadjuvant
chemo-radiotherapy. MSI state can be influenced by
chemo-radiotherapy. A qualification of MSI as a predictor for a
possible chemoresistance against 5-FU becomes apparent by the
results of the CAO/ARO/AIO-04 trial. This is a new source of
evidence for prognosis and progression of disease for patients with
rectal carcinomas. | de |
