Molekulare Zellstressmechanismen bei der hereditären Einschlusskörpermyopathie

dc.description.abstractMutationen im UDP-N-acetylglucosamine-2-epimerase/Nacetylmannosaminekinase (GNE)-Gen sind die kausale Pathogenese bei der hereditären Einschlusskörpermyopathie (hIBM) (Synonym: Nonaka Disease, distal myopathy with rimmed vacuoles [DMRV]). Neben der durch die Mutation im GNE-Gen bedingten Defiziten bei der Sialylierung sind die grundlegenden Pathomechanismen die zur Ausbildung der Muskelpathologie führen noch nicht verstanden. Es konnte gezeigt werden, dass bei der sporadischen Einschlusskörpermyositis (sIBM) den pro-inflammatorischen Zellstress-Mediatoren αB-Crystallin und Inducible Nitric Oxide Synthase (iNOS) eine Schlüsselrolle in der Pathologie zukommt. Ziel der vorliegenden Arbeit ist Zellstress und pro-inflammatorische Ereignisse ebenfalls im hIBM- Muskel nachzuweisen. Hierfür wurden 10 Muskelbiopsieproben von hIBM-Patienten auf die mRNA-Expression von Markern für Zellstress, Entzündung und β-amyloid untersucht und mit nicht-myopathischen Kontrollen verglichen. Mit immunhistochemischen, seriellen Doppelfärbungen wurden Skelettmuskelbiopsien für die Marker Amyloid Precursor Protein (APP), Major Histocompatibility Complex (MHC)-I, αB-Crystallin, Neuronales Zell Adhäsions Molekül (NCAM), Interleukin (IL)-1β, β-amyloid, iNOS, und phosphorylierte Neurofilamente (Pneurofilament) sowie Hämalaun/Eosin gefärbt. Insgesamt wurden 2871 Muskelfasern aus korrespondierenden Arealen von allen Biopsien quantitativ ausgewertet. Zudem wurden die Ergebnisse nach Unterschieden zwischen den Geschlechtern untersucht. Alle untersuchten Marker waren nachweisbar und die mRNA-Expression von APP, NCAM, iNOS, TNF-α und TGF-β war höher bei der hIBM verglichen mit den Kontrollen, allerdings erreichte dies keine statistische Signifikanz. Die mRNA-Expression von APP und αB-Crystallin korrelierte signifikant mit der Expression einiger pro-inflammatorischer und Zellstress-assoziierter Marker wie IL-6, MHC-I, CCL-3 und CCL-4. Bei der Immunhistochemischen Analyse zeigte sich eine Ko-Hochregulation von αB-Crystallin und iNOS und die Anzahl an Fasern positiv für αB-Crystallin, NCAM, MHC-I und iNOS korrelierte signifikant miteinander. Eine große Anzahl an Fasern positiv für αB-Crystallin war doppelt positiv für iNOS (23.5+-6.2%) und andersherum (16.8+-3%). Einerseits waren zahlreiche morphologisch auffällige Fasern positiv für αB-Crystallin und iNOS, zum anderen zeigten aber auch einige normal erscheinende Muskelfasern eine Überexpression für αB-Crystallin und iNOS. Es konnte keine signifikanten Unterschiede in der mRNA-Expression und Proteinexpression zwischen weiblichen und männlichen hIBM-Patienten- Proben nachgewiesen werden. Diese Daten weisen eine Korrelation zwischen der mRNA-Expression degenerativer und zellstress-assoziierter Marker mit der von pro-inflammtorischen Markern nach und zeigen, dass die Zellstress-assoziierten Moleküle αB-Crystallin und iNOS überexprimiert sind. Dies gibt möglicherweise Hinweise auf frühe Pathomechanismen bei der hIBM. für αBcrystallin, NCAM, MHC-I und iNOS korrelierte signifikant miteinander. Besides GNE-dependent deficits in sialylation, the underlying pathomechanisms that lead to damage of the muscle fibres have remained elusive. In sporadic inclusion body myositis (sIBM), the pro-inflammatory cell stress mediators αB-crystallin and inducible nitric oxide synthase (iNOS) have been shown to be relevant to the disease pathology. These data aim to identify pro-inflammtory and cell stress events in hIBM muscle. Therefore 10 muscle biopsies from hIBM patients were analyzed for mRNA-expression of markers of cell stress, inflammation and β-amyloid and compared to nonmyopathic controls. Using double-labeling immunohistochemistry, serial sections of skeletal muscle biopsies were stained for amyloid precursor protein (APP), major histocompatibility complex (MHC)-I, αB-crystallin, neural-cell-adhesion-molecule (NCAM), interleukin (IL)-1β, β-amyloid, iNOS, and phosphorylated neurofilament (Pneurofilament) as well as hematoxylin/eosin histochemistry. Corresponding areas of all biopsies with a total of 2,817 muscle fibres were quantitatively assessed for all markers. Furthermore the results were tested for differences between genders. All markers were detectable and the mRNA-expression of APP, NCAM, iNOS, TNF-α and TGF-β was higher in hIBM compared to controls, yet this did not reach statistical significance. The mRNA-expression of APP and αB-crystallin significantly correlated with the expression of several pro-inflammatory and cell-stress associated markers as IL6, MHC-I, CCL-3, and CCL4. As reflected by immunohistochemistry, αBcrystallin and iNOS were co- upregulated and the number of fibres positive for αBcrystallin, NCAM, MHC-I and iNOS significantly correlated with each other. A large fraction of fibres positive for αB-crystallin were double positive for iNOS (23.5+-6.2%) and vice versa (16.8+-3%). On the one hand, several fibres with structural abnormalities were positive for αB-crystallin and iNOS. On the other hand, particularly normal appearing fibres displayed an overexpression of αB-crystallin and iNOS. There was no significant difference between mRNA-expression and proteinexpression between the probes of female and male hIBM patients. In these data it is shown that the mRNA-expression of degenerative and cell stress molecules correlate with pro-inflammatory molecules in hIBM and the cell stress molecules αB-crystallin and iNOS are overexpressed. This may identify early disease mechanisms in hIBM muscle. The data help to better understand the pathology of hIBM.de
