dc.description.abstract | Im Zuge der medizinischen Fortschritte,
die im vergangenen Jahrhundert bei den operativen Techniken und
Zugangswegen von Nierentransplantationen, Prostatektomien sowie
Lymphadenektomien erreicht wurden, traten auch neue Komplikationen
wie Lymphozelen auf. Zielführend in der vorliegenden retrospektiven
Arbeit war dabei die Hypothese über einen Zusammenhang zwischen den
Konzentrationen der Interleukine 6 und 8 (IL-6, IL-8) sowie der
möglichen Keimbesiedlung von postoperativ aufgetretenen
Lymphozelen. Aufgrund dieser Überlegung konnten im Zeitraum
zwischen 1998 und 2006 die Daten von 60 zumeist
nierentransplantierten Patienten erhoben werden. Dazu erfolgte
sowohl eine konservative mikrobiologische Diagnostik als auch die
laborchemische Untersuchung der Blutproben. Mittels ROC-Analyse
wurde im Anschluss die Aussagefähigkeit von IL-6 und IL-8
ermittelt, graphisch dargestellt sowie Cut-off- Punkte festgelegt,
die eine Entscheidung bezüglich einer Infektion ermöglichen sollen.
In Zusammenschau der Ergebnisse wiesen Patienten mit
mikrobiologisch nachgewiesener Infektion tatsächlich erhöhte
Zytokinkonzentrationen auf. Auch das Merkmal der Signifikanz wird
hier evident. Lediglich in der Subgruppenanalyse der weiblichen
Studienteilnehmer ist dies, vermutlich aufgrund der geringen Zahl
an Patientinnen, nicht gelungen. Dennoch zeigen sich auch hier
klare Tendenzen in Richtung der Bestätigung unserer Hypothese.
Zusammenfassend kann festgestellt werden, dass die Zytokine IL-6
und vor allem IL-8 ihren Wert zur Beurteilung einer etwaigen
Infektion einer Lymphozele im vorliegenden Patientenkollektiv
nachgewiesen haben. Mit Hilfe der gefundenen Cut-off-Punkte konnten
zudem gute Entscheidungsgrundlagen geschaffen werden, um die
entsprechend notwendige Therapie zügig einleiten zu können. Dennoch
müssen die Ergebnisse in multizentrischen Studien mit deutlich
höheren Patientenzahlen bestätigt werden. | de |

dc.description.abstracteng | In terms of both improved operative
techniques and surgical approaches of kidney transplantations,
radical prostatectomies as well as lymphadenectomies, achieved
during the last century, there have also been new complications
such as lymphoceles. Our aim was to investigate if there is a
connection between the concentration of the cytokines IL-6 and IL-8
and a possible postoperative infection of these lymphoceles.
Between 1998 and 2006 we were able to evaluate the information of
60 patients who underwent a transplantation of the kidney in most
cases. As clinical parameters we took the conservative
microbiological diagnostics as well as patients' blood samples into
consideration. By means of ROC-analysis and several charts we
assessed the value of IL-6 and IL-8 and defined cut-off-points. As
a result those patients with an infected lymphocele really had
significantly higher concentrations of IL-6 and IL-8. However, the
subgroup analysis among the female patients could not generate the
same clear results due to just a small amount of women within the
collective. Nevertheless, even these non-significant results tend
to verify our hypothesis. Finally, we can summarize that both IL-6
and most of all IL-8 have proven their value in assessing a
possible infection of a lymphocele in our patients. By use of the
cut-off-points we could provide a solid base to initiate rapidly a
certain therapy. Though, these results need to be confirmed in
larger multicenter studies with a higher number of patients
