Cisplatin is often used in chemotherapy for the potentially curative treatment of different types of cancer. Associated nephrotoxicity in the process is an often observed problem to deal with which can limitate the treatment. An early detection of an restricted renal function is necessary to modulate the treatment well-timed. The intent of this study was to evaluate cystatin C as an easy, fast available and exact marker concerning this matter. 32 female and 56 male patients aged between 17 and 76 (mean age 57.6 years) who received an Polychemotherapy including cisplatin because of a malignant underlying disease were studied. To estimate renal function six different methods of determination were used (serum creatinine, serum cystatin C, Cockcroft-Gault-equation, MDRD formula, Hoek formula and Larsson formula). Blood samples were taken before and after treatment. Cystatin C (19,6%) increased higher than serum creatinine (9.6%) after treatment with cisplatin. ROC curves were determined and we found a significant difference between the diagnostic accuracy in favour of serum cystatin C compared to serum creatinine (P <0.01). Both cystatin C-based equations showed a significant decrease after apllied polychemotherapy and therefore a potential retricted renal function (P <0.01). Regarding creatinine-based equations only the MDRD-formula was found to decrease after therapy (P =0.03) whereas the Cockcroft-Gault-equation just showed an unsignificant decrease (P =0.07). The correlation of the respective formulas to each other, in detail creatinine clearance according to MDRD formula (P <0.0001) and Larsson formula according to Hoek formula (P <0.0001) were highly significant. Larsson formula (vs. MDRD: r =0.53 before and r =0.57 after treatment, r =0.46 for the difference; vs. Cockcroft-Gault-equation: r =0.42 before and r =0.46 after treatment, r =0.43 for the difference) correlated as well as Hoek formula (vs. MDRD: r =0.54 before and r =0.58 after treatment, r =0.48 for the difference: vs. Cockcroft-Gault-equation: r =0.43 before and r =0.47 after treatment, r =0.45 for the difference) more with the MDRD formula than with the Cockcroft-Gault-equation.

Tumorleiden eingesetzt. Die Cisplatin-induzierte Nephrotoxizität
ist dabei ein beobachtetes Problem, welches oftmals zu
Einschränkungen in der Therapie führt. Eine frühzeitige Erkennung
einer eingeschränkten Nierenfunktion ist daher notwendig, um
potientielle Modifikationen der Therapie rechtzeitig vornehmen zu
können. Diese Studie sollte zeigen, dass Cystatin C dabei ein
leichter, schnell verfügbarer und genauer Marker sein kann. Es
wurden 32 Frauen und 56 Männer im Alter von 17-76 Jahren
(Durchschnittsalter: 57,6 Jahre), welche eine Cisplatin-haltige
Polychemotherapie bei malignen Grunderkrankungen erhielten
untersucht. Um die Nierenfunktion zu bestimmen wurden sechs
verschiedene Bestimmungsmethoden der GFR (Kreatinin im Serum,
Cystatin C im Serum, Cockgroft-Gault-Formel, MDRD-Formel,
Hoek-Formel, Larsson-Formel) vor und nach der Gabe der
Polychemotherapie untersucht und miteinander verglichen. Cystatin C
zeigte einen signifikanteren Anstieg (19.6%) als dies beim
Kreatinin im Serum (9.6%) der Fall war. Die ROC-Analyse zeigte
einen signifikanten Unterschied zwischen der diagnostischen
Aussagekraft zugunsten von Cystatin C im Serum im Vergleich zu
Kreatinin im Serum eine pathologische Kreatininclearance (<78
ml/min) zu erkennen (P <0.01). Beide Cystatin C-basierenden
Formeln zeigten nach der erhaltenen Chemotherapie einen Abfall der
GFR und somit eine mögliche Nierenfunktionsstörung frühzeitig an
(Hoek-Formel P<0.01, Larsson-Formel P<0.01). Bei den
Kreatinin-basierenden Methoden registrierten wir lediglich bei der
MDRD-Formel einen signifikanten Abfall der GFR (P=0.03), während
der leichte Abfall der Kreatininclearance nach Cockgroft und Gault
nicht signifikant (P=0.07) war. Die jeweiligen Formeln, die
Kreatininclearance nach Cockgroft und Gault mit der MDRD-Formel
(r=0.81 vor und r=0.80 nach Chemotherapie, r=0.99 bei der
Differenz) und die jeweiligen Formeln nach Larsson und Hoek (r=0.99
vor und r=0.99 nach Chemotherapie, r=0.99 bei der Differenz),
korrelierten sehr stark untereinander. Die Larsson-Formel (vs.
MDRD: r=0.53 vor und r=0.57 nach Chemotherapie, r=0.46 bei der
Differenz: vs. Cockgroft und Gault: r=0.42 vor und r=0.46 nach
Chemotherapie, r=0.43 bei der Differenz) korrelierte genauso wie
die Hoek-Formel (vs. MDRD: r=0.54 vor und r=0.58 nach
Chemotherapie, r=0.48 bei der Differenz: vs. Cockgroft und Gault:
r=0.43 vor und r=0.47 nach Chemotherapie, r=0.45 bei der Differenz)
stärker mit der MDRD als mit der Kreatininclearance nach Cockgroft
und Gault.
