dc.description.abstract | Die CMD ist ein weitverbreitetes
Krankheitsbild in der Gesellschaft und bedarf aufgrund ihrer
Komplexität individueller sowie effektiver Therapieformen. So ist
es auch nicht weiter verwunderlich, dass durch stetig neue
technische Erkenntnisse auch zukünftig zielgerichtete Studien das
therapeutische Spektrum erweitern müssen. Daher beschäftigte sich
die vorliegende Studie mit der Frage, ob behandlungsbedürftige
Patienten mit ADVmR durch die Hilfe eines neuartigen
multifunktionellen Schienentherapiekonzeptes zu therapieren sind
und inwieweit die Befundung von realen Unterkieferbewegungen in die
Bewertung zu integrieren ist. So konnte nach zahntechnischer
Herstellung dreier verschiedener dentaler Aufbissschienen eine
vergleichende Untersuchung mit Hilfe der real-time-MRT und
überlagerten dynamischen Sequenzen durchgeführt werden. Diese
Studie konnte durch ihre Ergebnisse die Wirkweise einer
bimaxillären Kalottenschiene bei ADVmR belegen und bestätigte den
klinischen Mehrwert einer Verwendung von MRT-Sequenzen in Echtzeit.
Trotz der vielversprechenden Ergebnisse einer bimaxillären
Kalottenschiene sind fortlaufende Studien nötig, um die Wirkweise
dieser Schiene weiter zu untersuchen und eine generelle
Therapieempfehlung aussprechen zu können. Obwohl die
herausgearbeiteten Vorteile einer real-time-MRT ersichtlich waren,
ist abzuwarten, ob sie sich auch in der täglichen instrumentellen
Funktionsanalyse bewähren kann. Durch das breite therapeutische
Spektrum der bimaxillären Kalottenschiene und die Verwendung einer
real-time-MRT könnten sich neue Möglichkeiten bei einer Behandlung
von ADVmR ergeben. | de |

dc.description.abstracteng | CMD is a widespread clinical pattern in
society and due to its complexity it needs individual as well as
effective forms of therapy. It comes as no surprise that because of
steady new technical findings, in the time to come target-oriented
studies must expand the therapeutic spectrum. Therefore the study
at hand is concerned with the following question: can patients with
anterior disc displacement with reduction (ADDwR) be treated with
the help of an innovative kind of multifunctional splint therapy
and to what extent should the diagnosis of real mandible movements
be integrated into the evaluation. Consequently it was possible to
conduct a comparative study with the help of real time MRI and
overlaid dynamic sequences after the dental manufacture of three
different occlusal splints. This study's results could prove the
mechanism and the effect of a bimaxillary calotte splint on ADDwR
and validate the clinical value of real time MRI sequences. Despite
the promising results achieved with the bimaxillary calotte splint,
successive studies are necessary to examine the mode of impact of
this kind of splint and to be able to issue a general therapy
recommendation. Although advantages of real time MRI were evident,
it remains to be unknown if that method will also prove itself in
daily instrumental function analysis. The wide therapeutic spectrum
of bimaxillary calotte splint and the use of real time MRI open up
new possibilities in the treatment of ADDwR. | de |
