dc.description.abstract | Bei der Mehrzahl der Patienten mit
chronischer myeloischer Leukämie (CML) induziert die Inhibition der
BCR-ABL-Tyrosinkinaseaktivität durch die spezifischen
BCR-ABL-Inhibitoren (TKI) Imatinib, Nilotinib und Dasatinib eine
vollständige Remission der Erkrankung. Allerdings kommt es nach dem
Absetzen der Therapie oder dem Erwerb einer Resistenz-vermittelnden
Mutation des bcr/abl-Gens zu einem Rückfall, ausgehend von einer
kleinen Zellfraktion, die während der Primärtherapie persistieren
konnte. In Vorarbeiten zu leukämischen Vorläuferzellen mit dem
Phänotyp der „side population“ (SP) wurde bereits gezeigt, dass
CD34-positive Vorläuferzellen der CML eine hohe Expression des
intrazellulären ABC-Transporters A3 (ABCA3) an den Membranen der
Lysosomen aufwiesen, und dass Imatinib innerhalb der Leukämiezellen
zu einem großen Anteil (>75 %) lysosomal kompartimentiert
vorlag. Im Rahmen dieser Doktorarbeit wurden daher die
Expressionsregulation von ABCA3 in Leukämiezellen sowie der
Mechanismus des ABCA3-vermittelten zellulären Exports der
Tyrosinkinase-Inhibitoren näher untersucht. Mittels ektoper
Überexpression sowie shRNA-mediierter Suppression wurde
nachgewiesen, dass ABCA3 leukämische Zellen vor dem zytotoxischen
Effekt der TKIs Imatinib, Dasatinib und Nilotinib schützte. In den
die TKI-Exposition überlebenden Zellen steigerte die TKI-Behandlung
die Expression von ABCA3 in vivo und in vitro, assoziiert mit einer
gesteigerten Expression des embryonalen Transkriptionsfaktors
SALL4, der an den ABCA3-Promotor bindet. Inhibition von ABCA3 oder
SALL4 mittels genetischer Manipulation oder den Substanzen
Indomethacin und Rapamycin, allerdings nicht Gamma-Interferon
unterbrach die SALL4-abhängige Regulation von ABCA3 und stellte die
Suszeptibilität der leukämischen Zellen gegenüber TKIs wieder her.
In weiteren Analysen war dann nachzuweisen, dass Imatinib nach
lysosomaler Sequestrierung sowohl als freie Substanz sowie
angereichert in Mikrovesikeln vereinbar mit sekretorischen
Lysosomen aus den leukämischen Zellen exportiert wurde. Die
Ergebnisse dieser Arbeit können zu der Entwicklung von Strategien
beitragen, die durch Begleittherapien den Anteil des
intrazellulären freien Tyrosinkinase-Inhibitors erhöhen und damit
eine Verbesserung der Behandlung von CML-Patienten erreichen

dc.description.abstracteng | Inhibition of the BCR-ABL tyrosine kinase
activity by the specific tyrosine kinase inhibitors (TKI) imatinib,
nilotinib and dasatinib leads to complete remission of the disease
in most patients with chronic myeloid leukemia (CML). Cessation of
CML treatment or the occurrence of resistance mediating bcr/abl
gene mutations leads to relapse of the disease, initiated by a
small fraction of cells that persisted during CML therapy. Those
cells are CD34 positive leukemic progenitor cells with the
phenotype of the "side population" and show high expression of the
intracellular ABC transporter A3 (ABCA3) at the lysosomal
membranes. The majority (>75 %) of imatinib in these cells was
sequestrated in the lysosomal compartment. The aims of this work
were the examination of the ABCA3 expression regulation and of the
mechanism leading to ABCA3 mediated cellular export of TKI in
leukemic cells. Overexpression and shRNA mediated suppression of
ABCA3 showed that this transporter protects leukemic cells from the
cytotoxic effects of imatinib, dasatinib and nilotinib. In those
cells surviving the exposition to TKI, the expression of ABCA3 was
highly increased in vivo and in vitro, associated with a strong
increase of the transcription factor SALL4 that is able to bind to
the ABCA3 promoter. Inhibition of ABCA3 or SALL4 by genetic
manipulation or inhibiting substances indomethacin or rapamycin,
but not gamma interferon disrupted the SALL4-dependent regulation
of ABCA3 expression and restored the susceptibility of the leukemic
cells towards TKI treatment. Further analysis showed that imatinib
accumulated in microvesicles after lysosomal sequestration and was
exported out of the cells. The results of this work may lead to the
development of strategies to increase the amount of intracellular
active TKI and thereby improve the treatment of CML patients. | de |
