dc.description.abstractIn der vorliegenden Arbeit wurde der Nachweis disseminierter Tumorzellen im peripheren Blut und Knochenmark von Mammakarzinompatientinnen immunzytochemisch unter Verwendung monoklonaler Antikörper gegen Zytokeratin 18 sowie mittels einer nested RT-PCR zum Nachweis von PTHrP-Transkripten untersucht. Im Knochenmark und peripheren Blut einer Kontrollgruppe von 8 Patientinnen mit einer benignen Erkrankung der Mamma wurden in 2 von 8 KM-Proben und in keiner der PB-Proben CK-18-positive Zellen nachgewiesen. Die positiven KM-Proben stammten von Patientinnen mit einer Mastopathie, adenoiden oder papillomatösen Veränderungen der Brustdrüse. Für den Nachweis von PTHrP mittels der RT-PCR stellten sich 7 KM- und PB-Proben negativ dar. Innerhalb eines mittleren Beobachtungszeitraums von 30 Monaten ergab sich für diese Patientinnen kein Hinweis auf die Entwicklung eines epithelialen Tumors. Proben von 46 Mammakarzinompatientinnen wurden untersucht, von denen 41 ein primäres Mammakarzinom und 5 ein Rezidiv ihrer Erkrankung zum Zeitpunkt der Probenentnahme aufwiesen. Der mediane Beobachtungszeitraum nach Analyse betrug 20 Monate. Mit der APAAP-Färbung konnte ein positiver Nachweis von CK 18 in 9 von 37 KM und 4 von 37 PB in Zytozentrifugationspräparaten, bzw. in 5 von 14 KM und 5 von 13 PB in Sedimentationspräparaten geführt werden. In der nested RT-PCR wiesen 10 von 34 Patientinnen im KM und 9 von 35 Patientinnen im PB sowie alle 12 Tumorgewebe ein positives Ergebnis für PTHrP auf. Eine Korrelation mit den etablierten Prognosefaktoren der TNM-Klassifikation sowie dem Östrogen- und Progesteronstatus wurde nicht festgestellt. Positive Nachweise mit allen drei Methoden lagen vornehmlich bei Patientinnen mit niedrigeren Tumorstadien vor, wobei die Ergebnisse nur partiell konkordant waren. Für keine der untersuchten Methoden war eine Assoziation mit einem verkürzten rezidivfreien Intervall oder einem verkürzten Überleben der Patientinnen festzustellen. Die Bedeutung der Nachweise CK-18-positiver Zellen in den Proben von Patientinnen mit einer benignen Erkrankung der Mamma und insbesondere die Bedeutung der relativ hohen Nachweisraten disseminierter Tumorzellen der nodal negativen Mammakarzinompatientinnen mit kleinen Tumoren bedarf weiterer Abklärung.de

dc.description.abstractengDetection of disseminated tumour cells in peripheral blood (pb) and bone marrow (bm) of patients with breast cancer was performed by immunocytology using monoclonal antibodies specific for cytoceratin 18 as well as employing a nested RT-PCR for the detection of transcripts of PTHrP. As controls, bone marrow and peripheral blood of 8 patients with benign breast diseases were investigated. Two of eight bone marrow samples and none of the peripheral blood samples stained positive for CK 18. These samples had been obtained from patients with mastopathia, adenomas and papillomas of the breast. The samples of this cohort were tested negative for PTHrP expression. Within a mean follow-up of 39 months, none of these patients developed an epithelial tumour. Samples of 46 patients with breast cancer were investigated, 41 at primary diagnosis, and 5 at the time of relapse. The mean follow-up time after immunocytological and molecular investigation were 20 months each. Using the Alkaline Phosphatase-Anti-Alkaline Phosphatase (APAAP) for CK 18 on cytospins, 9 of 37 bone marrow samples and 4 of 37 samples of peripheral blood stained positive. In samples prepared with the sedimentation technique, 5 of 14 samples of bone marrow and 5 of 13 samples of peripheral blood showed positive results. Employing the nested RT-PCR 10 of 34 bm, 9 of 35 pb, and all 12 tissues of the primary tumours expressed the PTHrP-gene. Positive results by any of the three methods investigated were not associated with adverse prognostic parameters, such as advanced disease according to TNM-classification or negative receptor status. Positive results in all three methods showed only a partial overlap and were observed mainly in samples of patients with a lower state of disease according to the TNM-system. None of the positive results in any of the three methods suggested a shorter time to progression or a shorter overall survival. The significance of the relatively high incidence of positive results for CK 18 in patients with benign diseases of the breast, as well as the high detection rate of disseminated tumour cells in patients with node-negative breast cancer with small tumours warrant further investigation.de
