dc.description.abstract | Die massive infektionsbedingte Schädigung
der mukosalen Gewebe, sowie die vorwiegend sexuelle Übertragung von
HIV über mukosa-assoziierte Gewebe erfordern genauere Kenntnisse
dieser Organe. Die vergleichende durchflusszytometrische Analyse
neun verschiedener mukosaler Organe in nicht infizierten
Rhesusaffen wies hinsichtlich mehrerer Lymphozytenpopulationen
wesentliche Unterschiede der Gewebe untereinander und im Vergleich
zum Blut auf sowie ein unterschiedlich ausgeprägter Verlust an CD4+
T-Zellen und virusspezifischer Immunantwort während der
SIV-Infektion. Die umfangreiche Analyse einer großen Kohorte von
LTNPs ergab, dass die Besonderheit dieser klinisch unauffälligen
SIV-infizierten Tiere in der langfristigen systemischen und
mukosalen Kontrolle der Viruslast liegt. Dies geht einher mit
physiologischen Anteilen an CD4+ T-Zellen und einer geringen
Immunaktivierung in Blut und verschiedenen mukosalen Geweben. Zudem
leistet offensichtlich eine starke virusspezifische polyfunktionale
mukosale Immunantwort einen wesentlichen Beitrag zur
Aufrechterhaltung des LTNP-Status und zur Repopulation der
mukosalen CD4+ T-Zellen. Die Langzeitanalyse SIV-infizierter
Rhesusaffen lieferte deutliche Hinweise, dass ein Anstieg der
RNA-Viruslast im Blut nicht zwangsläufig mit der Entstehung von
AIDS-ähnlichen Symptomen assoziiert ist. Vor allem der komplette
Verlust systemischer und mukosaler virusspezifischer T-Zellen sowie
der Anstieg an Zellen, die infektiöse Partikel produzieren, sind
offensichtlich von größerer prognostischer Bedeutung. Mit Hilfe des
RT-SHIV-Infektionsmodells unter Verwendung chinesischer Rhesusaffen
fand die erste Wirksamkeitsüberprüfung zur Entwicklung eines neuen
potenten Mikrobizides, in Form eines Vaginalgels basierend auf
einem NNRTI gegen HIV, statt. Es konnte mit einer
Wirkstoffkonzentration von 0,5% ein Schutz vor vaginaler
RT-SHIV-Infektion in 50-60% der damit vorbehandelten Tiere
festgestellt werden. Da durch zahlreiche Impfstoff-Experimente eine
sterile Immunität gegen HIV in den letzten 25 Jahren nicht möglich
war, sollte gerade ein Schutz vor sexueller Übertragung zukünftig
weiter im Vordergrund stehen. Dies sollte durch die
Weiterentwicklung potenter Mikrobizide erfolgen. Weiterhin sollten
künftige präventive und therapeutische Vakzinen unbedingt darauf
abzielen, das hier festgestellte natürliche Potential der mukosalen
Gewebe zur virusspezifischen Immunantwort zu stimulieren, um so die
mukosale Übertragung von HIV zu reduzieren und die Sterberate bei
HIV-Patienten zu senken. | de |

dc.description.abstracteng | The massive damage of mucosal tissue as a
consequence of infection as well as the sexual transmission of HIV
via mucosa-associated tissue requires a better knowledge about
these organs. The comparative flow-cytometric analysis of nine
different mucosal organs of uninfected rhesus monkeys showed
significant differences as to various lymphocyte populations of the
tissues among themselves and in comparison to blood. Additionally,
the loss of CD4+ T-cells and the virus-specific immune response
during the SIV infection differs considerably. The comprehensive
analysis of a large cohort of LTNPs showed that these clinically
healthy SIV-infected animals developed a long-term systemic and
mucosal control of the viral load. This is accompanied with
physiological parts of CD4+ T-cells and a low immune activation in
blood and in different mucosal tissues. Furthermore, a strong
virus-specific polyfunctional mucosal immune response is obviously
also responsible for the stable LTNP status and for the
repopulation of mucosal CD4+ T-cells. The long-term analysis of
SIV-infected rhesus monkeys clearly showed that an increasing RNA
viral load in blood is not inevitably associated with the
development of AIDS-like symptoms. Obviously the complete loss of
systemic and mucosal virus-specific T-cells as well as an increase
in cells producing infectious particles is of more prognostic
importance. The first efficacy trial for the development of a new
potent microbicide in the form of a vaginal gel based on a NNRTI
against HIV was performed in a RT-SHIV infection models using
rhesus monkeys of Chinese origin. 50-60% of the pretreated animals
were protected against a vaginal RT-SHIV infection after
application of a component concentration of 0.5%. During the last
25 years a sterile immunity against HIV was not achieved in various
vaccine experiments. Therefore, more attention should be directed
to a protection against sexual transmission. This should be done by
a further development of potent microbicides. Furthermore, it is
important that future preventive and therapeutic vaccines should
stimulate the natural potential of the mucosal tissues to the
virus-specific immune response. Thus, the mucosal transmission of
HIV may be reduced and the mortality rate of HIV-patients may
