dc.description.abstract | Dysphagien finden sich sowohl in der
Hals-Nasen-Ohrenheilkunde als auch in der Gastroenterologie. Das
Patientenkollektiv der vorliegenden retrospektiven Analyse bestand
aus 48 Patienten mit Dysphagie unterschiedlicher Genese. Aufgrund
einer Dysfunktion im Bereich des oberen Ösophagussphinkters konnten
17 Patienten im Mittel mit 45 U Botulinumtoxin A behandelt werden.
Infolge einer Achalasie konnten 31 Patienten im Mittel mit 100 U
Botulinumtoxin A im Bereich des unteren Ösophagussphinkters
behandelt werden. Bei 75,6% der Patienten (n=45) konnte eine
Verbesserung der Symptomatik im Mittel innerhalb der ersten beiden
postinterventionellen Tage nachgewiesen werden. Nach drei bis sechs
Monaten stellten sich die Patienten, aufgrund eines Nachlassens der
Wirkung, wieder vor. Aufgrund zahlreicher Begleiterkrankungen und
Dauermedikamente zeigte sich ein morbides Patientenkollektiv, das
vor allem an Herzkreislauferkrankungen litt. Das hier angewendete
Verfahren hat sich im Vergleich mit anderen Therapieoptionen als
einfach und komplikationsarm erwiesen. Schwere und dauerhafte
unerwünschte Wirkungen fehlen gänzlich. So stellt Botulinumtoxin A
vor allem bei morbideren und älteren Patienten eine gute
Therapieoption dar. Durch die reversible Wirkung an der motorischen
Endplatte können ebenso transiente wie chronische Dysphagien
effektiv behandelt werden. Die Wirkung von Botox® bei Dysphagie
wird bereits seit 1994 beobachtet und konnte seitdem mehrfach
nachgewiesen werden. Die vorliegenden Ergebnisse bestätigen den
therapeutischen Nutzen einer intramuskulären Injektion von
Botulinumtoxin A zur Behandlung der Dysphagie. | de |

dc.description.abstracteng | Different modes of dysphagia can be found
either in the oto-rhino-laryngology or in the gastroenterology. The
patient population in this retrospective analysis consists of 48
patients with dysphagia of different reasons. Due to a dysfunction
in the area of the upper esophageal sphincter 17 patients were
treated with a median dosage of 45 U botulinumtoxin A. Due to an
achalasia in the area of the lower esophageal sphincter 31 patients
were treated with a median dosage of 100 U. After a median duration
of two days after the intervention 75,6% of the total population
(n=45) noticed an improvement regarding the symptoms. After three
to six months most of the patients presented again because of a
decrease of the effect of botulinumtoxin. Due to a lot of
concomitant diseases and long-term medication, especially
cardiovascular diseases, the patient population showed to be
morbid. The therapy method adopted here turned out to be relatively
simple and low-risk in comparison with other therapy options.
Serious or continuous adverse effects did not arise. That is why
botulinumtoxin A is a good therapy option especially with morbid
and older patients. Because of its reversible effect on the
neuromuscular end-plate transient as well as chronic modes of
dysphagia can be treated with botulinumtoxin. Since 1994 the
effects of Botox® on dysphagia have been noticed and proved several
times. The presented results confirm the therapeutic advantage of
intramuscular injections of botulinumtoxin A as a therapy of
dysphagia. | de |
