Objectives: Implant-supported, bar retained overdentures in the edentulous maxilla and in the mandible were examined for their restoration- and their implant-related survival rates and the prevalence of technical and biological complications. Materials and methods: 36 implant-supported prostheses (12 in the maxilla/24 in the mandible) of 27 patients (16 female/11 male, mean age: 58.5±8.5 years), were investigated on the basis of file documentation and actual radiographs. The inclusion criteria were that the clinical follow-up was accomplished at least biannually and a functional period of 5 years or more. The re-examined constructions, supported by 161 implants (mean: 4.5 implants per jaw), had a mean clinical follow-up time of 7.3±3.6 years. Results: Two implants were lost (1.2%, the survival rate of the superstructures was 100%. Averagely 0.2 interventions per prosthetic restoration and year have to be performed to maintain clinical function. The implant-related periimplantitis rate was 12.4%, the patient-related one was 37%. Conclusions: Bar-retained overdentures show a high survival rate regarding the superstructures and the implants and constant low complication rates. On the other hand, biological complications like peri-implant bone loss, show high prevalence rates.

Retrospektive Analyse implantatgetragener, stegretinierter Deckprothesen im zahnlosen Kiefer Fragestellung: Implantatgetragene, stegretinierte Deckprothesen im zahnlosen Kiefer wurden hinsichtlich ihrer restaurations- und implantatbezogenen Überlebensraten sowie der Prävalenzraten technischer und biologischer Komplikationen untersucht. Material und Methode: 36 implantatgetragene Stegprothesen (12 im Ober- /24 im Unterkiefer) von 27 Patienten (16 weiblich/11 männlich; Durchschnittsalter bei Implantation 58,5±8,5 Jahre), wurden anhand der Behandlungsdokumentation und aktueller Orthopantomogramm-Aufnahmen nachuntersucht. Einschlusskriterien waren eine mindestens halbjährlich erfolgte klinische Nachuntersuchung sowie eine minimal prothetische Funktionsperiode von 5 Jahren. Die nachuntersuchten Konstruktionen mit 161 Implantaten (Mittelwert: 4,5 Implantate/Kiefer) wiesen eine mittlere klinische Beobachtungsdauer von 7,3±3,6 Jahren auf. Ergebnisse: Zwei Implantate gingen verloren (1,2%), die Überlebensrate der Suprakonstruktion betrug 100%, durchschnittlich 0,2 Interventionen pro prothetischer Restauration/Jahr mussten zum Funktionserhalt vorgenommen werden. Die implantatbezogene Periimplantitisrate betrug 12,4%, die patientenbezogene 37%. Schlussfolgerung: Stegretinierte Deckprothesen zeigen eine hohe Überlebensrate der Suprakonstruktionen und Implantate sowie eine konstant geringe prothetische Komplikationsrate. Demgegenüber zeigen biologische Komplikationen in Form von periimplantärem Knochenverlust hohe Prävalenzraten.
