To determine the rate of hernia recurrences after TAPP for all types of inguinal and femoral hernias in adults, complications and the satisfaction with the hernia repair, the records from 1993 to 2009 (7010 TAPP, 93.9% of all hernia repairs) were analyzed. For quantitative results, living patients, operated 2, 7, 12 and 17 years ago were asked by a questionnaire.We found 17 years after TAPP 3.4% recurrences, after 12 years 1.0%, after 7 years 2.4% and after 2 years 0.4% . 60.5% of recurrences developed within 2 years postoperatively, 15.8% after 5 years and 4.0% later than 10 years. The relapse rate after TAPP for primary hernias was 2.3% and for recurrences after anterior repair 1.9%. 96.9% of all patients were satisfied with the result, only 1.5% was dissatisfied with the TAPP procedure, corresponding to recurrences, surgical complications and long-lasting inguinal/scrotal pain. TAPP can be used as standard procedure in complex hernias (scrotal hernias, incarcerated hernias and hernias after radical prostatectomy) too without increased recurrence rates, but with longer operation-times and elevated surgical complications (large scrotal hernias).
