dc.description.abstractDie objektive Stimmanalyse ist für die tägliche phoniatrische Praxis von grundlegender Bedeutung bezüglich der Diagnostik und Therapie von Stimmstörungen. Als Goldstandard gilt das Göttinger Heiserkeits-Diagramm (GHD), welches die Aufzeichnung von 28 Vokalen durch geschultes Fachpersonal mit einem zeitlichen Aufwand von ca. 15 Minuten pro Patient erfordert. In der vorliegenden Studie wurde untersucht, ob das GHD auch dann valide Ergebnisse für die Stimmqualität liefert, wenn statt des Standardprotokolls gehaltene Phonationen aus indirekter Laryngoskopie oder Videostroboskopie analysiert werden (sog. „reduziertes Protokoll“). Wäre dies der Fall, ließe sich Stimmanalyse und Untersuchung des Larynx in einem Arbeitsschritt durchführen und somit der zeitliche und personelle Aufwand deutlich reduzieren. Es wurden Stimmaufnahmen aus Stroboskopie und Laryngoskopie von 213 Patienten (97 männlich, 116 weiblich) mit Hilfe des GHD analysiert. Am gleichen Untersuchungstag erfolgte zudem eine typische Mikrophonaufnahme gehaltener Phonationen zur Analyse nach dem vollständigen GHD-Protokoll. Die aus reduziertem und vollständigem Protokoll ermittelten Werte für die Irregularität und die Rauschkomponente des Stimmsignals als objektive Marker der Stimmqualität wurden jeweils korreliert. Sowohl für die Irregularitätskomponente (r=0,65) als auch für die Rauschkomponente (r=0,55) ergaben sich signifikante Korrelationen (p<0,001) zwischen beiden Verfahren. Außerdem zeigte sich, dass bereits eine einzige Stimmgebung aus Laryngoskopie und Stroboskopie ein zuverlässiges Ergebnis liefert. Es konnte eine Mindesttonhaltedauer von 1 Sekunde ermittelt werden. Die Vereinfachung des Vokals während Laryngoskopie beeinflusst das Ergebnis nicht und beide Methoden eignen sich zur klinischen Verlaufskontrolle.de

dc.description.abstractengObjective voice analysis is of profound importance for daily phoniatric practices, for diagnostics and the therapy of dysphonia both. The Göttingen Hoarseness-diagram (GHD) is considered to be the ultimate benchmark. It requires recording 28 vocals by qualified personnel in ca. 15 minutes time. This study investigates wether the GHD is able to produce valid results for voice quality if, instead of standard protocol, phonations recorded during laryngoscopy and stroboscopy are analyzed (so-called “reduced protocol”). If this was possible, voice analysis and examination of the larynx could be performed simultaneously thus leading to a reduced expenditure of time and personnel. For this thesis, voice recordings taken during the stroboscopy and laryngoscopy of 213 patients (97 male, 116 female) were analyzed with the GHD. On the same day phonations following the complete GHD-protocol were also recorded and analyzed. The GHD measures the voice´s irregularity and noise component and the results for this markers, determined by reduced and complete protocol, were then correlated with each other. A significant correlation (p< 0,001) for both irregularity (r=0,65) and noise component (r=0,55) was found between the two methods. Furthermore, even the analysis of only one single phonation recorded during laryngoscopy and stroboscopy achieved a reliable result. The minimum duration of phonations was determined to be one second. The vocal’s simplification due to the laryngoscopy did not affect the result of voice analysis and both the reduced as well as the standard method can be used for follow-ups.de
