dc.description.abstractgerDie Therapie periprothetischer Frakturen ist aufgrund des hohen Alters und des häufig schlechten Gesundheitszustandes der Patienten sowie der Komplexität der Behandlung mit hohen Komplikationsraten verbunden. Aufgabe dieser Studie war es, Unterschiede in der Versorgung und im Komplikationsspektrum periprothetischer Femurfrakturen bei Patienten mit Hüft- und Kniegelenkersatz darzustellen. Dazu erfolgte die retrospektive Analyse von 54 Frakturen bei einliegender Hüftendoprothese und von 17 Frakturen bei einliegender Knieendoprothese, die zwischen 2004 und 2011 in der Klinik für Unfallchirurgie, Orthopädie und Plastische Chirurgie der Universitätsmedizin Göttingen behandelt wurden. In der Gruppe mit Hüftendoprothese lag das Durchschnittsalter bei 80,9 Jahren, und 64,8 % der Patienten waren weiblich. 51,9 % der Patienten wiesen eine ASA-Klasse von 3 oder 4 auf. Dagegen waren 76,5 % der Patienten mit Knieendoprothese weiblich und mit durchschnittlich 76,2 Jahre deutlich jünger als die Patienten mit Hüftendoprothese. Damit einhergehend wiesen sie einen besseren Gesundheitszustand auf. Nur 29,4 % dieser Patienten wurden der ASA-Klasse 3 und niemand der ASA-Klasse 4 zugeteilt. Die häufigste Frakturursache stellten in beiden Gruppen häusliche Stürze dar. Hier muss die Prävention periprothetischer Frakturen ansetzen. Die Frakturen nach Hüftprothesenimplantation wurden zu 13,0 % konservativ, zu 48,1 % osteosynthetisch und zu 38,9 % mit einem Schaftwechsel versorgt. 100 % der Frakturen nach Knieprothesenimplantation wurden mit Plattenosteosynthesen versorgt. Die Operationsdauer (Hüfte: 171 min; Knie: 165 min) und die perioperative Gabe an Erythrozytenkonzentraten (Hüfte: 5,8; Knie: 2,4) waren im Durchschnitt in der Patientengruppe mit einliegender Hüftendoprothese höher als in der Gruppe mit einliegender Knieendoprothese. Die Frakturversorgung zog in dieser Studie zu einem großen Teil Komplikationen und Revisionseingriffe nach sich. 33,3 % der Patienten mit Hüftendoprothese waren von chirurgischen Komplikationen betroffen. Am häufigsten traten Hüftkopfluxationen auf. Unter den Patienten mit Knieendoprothese zeigten 41,2 % chirurgische Komplikationen, wobei es sich am häufigsten um Refrakturen und Pseudarthrosen handelte. Zweiteingriffe wurden bei 22,2 % der Hüftprothesenpatienten nötig, wobei gerade Patienten mit langer Operationsdauer bei der Frakturversorgung betroffen waren. Dieses deutet darauf hin, dass sich diese Frakturen schon während der ersten Operation als komplex darstellten. 29,4 % der Knieprothesenfälle mussten revidiert werden. Während in der Knieprothesengruppe keine akut lebensbedrohlichen Komplikationen auftraten, betraf dies 20,4 % der Hüftprothesenpatienten. Die Klinikletalität lag in dieser Gruppe bei 13,2 % und damit höher als in der Literatur angegeben. Als Risikofaktoren konnten nur ein Alter von mehr als 85 Jahren und eine ASA-Klasse von mindestens 3 nachgewiesen werden. Die geringere allgemeine Komplikationsrate bei Patienten mit Knieendoprothesen kann auf den besseren Gesundheitszustand und die geringere Invasivität des Eingriffs gegenüber Patienten mit Hüftendoprothesen zurückgeführt werden. Langfristig sollten Studien mit größeren Fallanzahlen zum Komplikationsspektrum periprothetischer Frakturen erstellt werden. Da die Komplikationen in vielen Fällen mit dem hohen Alter der Patienten und ihren Vorerkrankungen zusammenhängen und eine Optimierung in der Therapie somit schwierig ist, muss ein Augenmerk auf die Vermeidung des Auftretens periprothetischer Frakturen im Sinne einer Sturzprävention und einer adäquaten Osteoporosetherapie gelegt werden.de
dc.description.abstractengThe therapy of periprosthetic fractures involves high complication rates due to patients' old age, their poor health and the complexity of medical treatment. The aim of this study was to identify differences in treatment and the diversity of complications in patients with periprosthetic femoral fractures following hip and knee arthroplasty. In a retrospective analysis 54 fractures following hip replacement and 17 fractures following knee replacement were investigated. All patients were treated in the clinic for accident surgery, orthopedics und plastic surgery at the University hospital Göttingen between 2004 and 2011. The average age of patients with hip arthroplasty was 80.9 whereof 64.8 % were female. 51.9 % were categorized in ASA class 3 or 4. In contrast 76.5 % of the patients with knee arthroplasty were female. With an average age of 76.2 these patients were clearly younger than patients of the hip group. Additionally, they showed a better state of health with only 29.4 % ASA class 3 and no ASA class 4 classifications. The most common reason for a fracture in either group was a fall at home. Prevention of periprosthetic fractures should therefore start at this point. 13 % of the fractures after hip replacement were treated non-operatively, 48.1 % were treated with osteosynthesis and 38.9 % were treated with a femoral stem revision. On the other hand, all fractures after knee replacement were treated with osteosynthesis. The average time of surgery (hip: 171 min; knee: 165 min) and the perioperative donation of erythrocyte concentrates (hip: 5.8; knee: 2.4) were both higher in the group of patients with hip arthroplasty than with knee arthroplasty. In this study the fracture treatment was associated with a high rate of complications and revision surgery. One third of the patients with a hip implant underwent surgical complications. The most common complication was a luxation of the femoral head. 41.2 % of the patients with a knee implant underwent surgical complications, most frequently refractures and pseudarthrosis. Revision surgery was necessary for 22.2 % of the patients with a hip implant, especially when the primary fracture surgery was time-consuming. This suggests a high complexity level of the initial fractures treated in the primary surgery. 29.4 % of the cases in the knee-implant group needed revision surgery. While no life-threatening complications occurred in the knee-implant group, 20.4 % of the patients in the hip-implant group had life-threatening complications. In this group a clinical mortality rate of 13.2 % was observed. This was higher than mortality rates quoted in literature. Only an old age of more than 85 years and an ASA class of 3 or more could be proved as risk factors in this study. The low general complication rate within patients of the knee-implant group can be explained by their better state of health as well as the lower grade of invasion of their surgical treatment. In the long term studies should be conducted with larger numbers of cases concerning the complications of periprosthetic fractures. Optimization of the therapy is difficult as many complications are associated with an old age and the medical history of the patients. In order to avoid periprosthetic fractures the focus should be on fall-prevention and an adequate therapy of osteoporosis.de
