dc.description.abstractgerSekundärleukämien nach Hochdosischemotherapien sind ein bekanntes Problem. Bisher ließ sich nicht eindeutig klären, wann leukämogene Klone entstehen und wie lange sie einer klinisch erfassbaren t-AML/MDS vorausgehen. Diese Studie sollte die Frage beantworten, ob ein prospektives Screening mittels PCR während und nach Chemotherapie sinnvoll ist, um eine sich entwickelnde Leukämie frühzeitig zu identifizieren und ob ein Zusammenhang zwischen dem Auftreten aberranter Zelle mit der Dosisintensität der Chemotherapie besteht. Im Hinblick auf diese Fragestellung wurden Non-Hodgkin-Patienten in mittlerer und hochdosierter Chemotherapie nach dem MegaCHOEP-Schema mit einer Kontrollgruppe in konventioneller CHOP-Therapie auf t-AML/MDS-assoziierte genetische Aberrationen t(9;22), t(8;21) und inv(16) mittels RT-PCR untersucht und miteinander verglichen. Bei den 65 Patientenproben der Studiengruppe wurden für Fusionstranskript BCR-ABL 3,1%, für AML1/ETO 0% und für CBFβ-MYH11 9,2% der Proben positiv getestet. Dem gegenüber steht die Kontrollgruppe mit 73 Proben; davon für t(9;22) 2,7%, für inv(16) 1,4% und t(8;21) 0%. Der vergleichsweise hohe Anteil der positiv auf inv(16) getesteten Proben hängt sehr wahrscheinlich mit der höheren Genotoxizität des CHOEP- bzw. MegaCHOEP-Therapieschemas gegenüber dem konventionellen CHOP-Protokoll zusammen. Die Häufigkeit therapieassoziierter Aberrationen ist viel größer als die der klinisch entstehenden therapieassoziierten Leukämien. Daher erscheint die prospektive molekulargenetische Diagnostik keine sinnvolle Maßnahme zur t-AML/MDS-Früherkennung zu sein.de
dc.description.abstractengSecondary leukemia following high dose chemotherapy are a well known problem in oncology. The appearance of leukemogenic clones and the time preceding a clinical detactable t-AML/MDS could not exactly be clarified to date. This study was designed to determine the value of a prospective screening via PCR performed during and after chemotherapeutic treatment to identify an evolving leukemia in an early stage. Furthermore the correlation between the appearance of aberrant clones and the treatment intensity should be investigated. In view of this question Non-Hodgkin patients in intermediate and high dose chemotherapy according to the MegaCHOEP scheme and a control group in conventional CHOP-therapy were tested for t-AML/MDS associated aberrations involving t(9;22), t(8,21) and Inv(16) using RT-PCR and compared. Positive findings were 3,1% for BCR-ABL, 0% of AML1/ETO and 9,2% for CBFβ-MYH11 out of 65 probes of the study group. Compared with the control group positive findings were 2,7% for t(9;22) ,1,4% for inv(16) and 0% for t(8;21) out of 73 probes. The comparatively high percentage of inv(16) positive findings in the study group is most likely connected to the increased genotoxicity of the CHOEP respectively MegaCHOEP scheme compared to the conventional CHOP protocol. The frequency of therapy induced aberrations is much higher than the clinical detected quantity of evolving leukemia. Therefore a prospective molecular genetic screening for t-AML/MDS does not seem to be a reasonable procedure.de
