dc.description.abstractger | Migräne ist mit einer weltweiten Prävalenz von 10% eine häufige Erkrankung und
führt alleine in Deutschland zu volkswirtschaftlichen Gesamtkosten von geschätzt
6,72 Mrd. Euro pro Jahr. Für die Behandlung der Migräne spielen Triptane eine
entscheidende Rolle. Am häufigsten wird in Deutschland das älteste Triptan Sumatriptan
verschrieben. Sumatriptan unterliegt einem hohen First-Pass-Effekt, der auf
einen ausgeprägten Lebermetabolismus zurückzuführen ist. Sumatriptan ist ein hydrophiles
Molekül, das nur begrenzt durch zelluläre Membranen diffundieren kann
und auf einen aktiven Transport angewiesen ist. Es liegt bei physiologischem pHWert
zu mehr als 95% in seiner protonierten Form als Kation vor. Der in der Leber
am stärksten exprimierte organische Kationentransporter ist der OCT1. Dieser ist genetisch
sehr polymorph und Varianten des OCT1 beeinflussen die Pharmakokinetik
und Wirkung von verschiedenen Medikamenten wie Metformin, Morphin, Tramadol
und Tropisetron.
In dieser Arbeit wurde untersucht, ob Sumatriptan einem hepatischen Transport
durch den humanen organischen Kationentransporter OCT1 unterliegt und ob Polymorphismen
des OCT1 einen Einfluss auf die Transportaktivität von Sumatriptan
haben. Darüber hinaus wurden Arzneimittel-Wechselwirkungen von typischen Komedikamenten
und Sumatriptan am OCT1 untersucht und der Transport anderer
Triptane durch den OCT1 betrachtet.
Für die Experimente wurden primäre humane Hepatozyten und HEK293-Zellen,
die den Wildtyp und Allelvarianten des OCT1 überexprimierten, verwendet. Es wurden
14 verschiedene Allelvarianten des OCT1 untersucht: OCT1-Allel *1 bis *7 und
*9 bis *15. Die Zellen wurden mit Sumatriptan inkubiert und die zelluläre Aufnahme
von Sumatriptan mittels HPLC-MS/MS quantifiziert. Die Aufnahme von Sumatriptan in primäre humane Hepatozyten lässt sich durch
das OCT1-Modellsubstrat MPP+ inhibieren. In hOCT1-überexprimierenden Zellen
ist die Aufnahme von Sumatriptan zeit- und konzentrationsabhängig und folgt einer
typischen Michaelis-Menten-Kinetik (KM = 55,4 ± 7,89 μM, vmax = 977,7 ±
131,6 pmol/mg Protein/min). Sumatriptan unterliegt einem Transport durch den
hepatischen organischen Kationentransporter OCT3. Die Transportleistung ist im
Vergleich zum OCT1 jedoch deutlich geringer (KM = 75,4 ± 6,15 μM, vmax = 90,2
± 20,28 pmol/mg Protein/min). Die OCT1-vermittelte Aufnahme von Sumatriptan
ist für die Allele OCT1*3 und OCT1*13 vermindert und für die Allele OCT1*4,
OCT1*5, OCT1*6, OCT1*12, OCT1*14 und OCT1*15 vollständig aufgehoben. Die
restlichen untersuchten Allele zeigen eine gegenüber dem OCT1*1-Wildtypallel nicht
veränderte Aufnahme von Sumatriptan. Der Sumatriptantransport durch OCT1 kann
durch die Migräneprophylaktika Amitriptylin (IC50 = 3,47 ± 0,08 μM) und Propranolol
(IC50 = 6,20 ± 0,06 μM) und die Antiemetika Domperidon (IC50 = 32,8 ±
7,44 μM) und Metoclopramid (IC50 = 15,9 ± 8,93 μM) inhibiert werden. Die Zweitgenerationstriptane
Naratriptan, Rizatriptan und Zolmitriptan unterliegen ebenfalls
einem Transport durch den OCT1. Dabei beobachtet man für Naratriptan die höchste
Transportrate. Es erreicht eine um Faktor 54,4-fach höhere Konzentration in OCT1-
überexprimierenden Zellen gegenüber Kontrollzellen.
Der organische Kationentransporter OCT1 ist ein high-capacity-Transporter von
Sumatriptan und für die Aufnahme von Sumatriptan in die Leber entscheidend. Polymorphismen
des OCT1 können diese Aufnahme stark beeinflussen, ebenso typische
Komedikationen in der Migränetherapie. Auch Zweitgenerationstriptane unterliegen
einem Transport durch den OCT1. | de |
dc.description.abstracteng | The low bioavailability of the anti-migraine drug sumatriptan is mainly caused by first-pass hepatic metabolism. I analyzed the impact of the hepatic organic cation transporter OCT1 and of OCT1 polymorphisms on sumatriptan cellular uptake. OCT1 transported sumatriptan with high capacity and sumatriptan uptake into human hepatocytes was strongly inhibited by the OCT1 inhibitor MPP(+) . Sumatriptan uptake was not affected by the Met420del polymorphism, but was strongly reduced by Arg61Cys and Gly401Ser, and completely abolished by Gly465Arg and Cys88Arg. OCT1 also transported naratriptan, rizatriptan, and zolmitriptan, suggesting a possible impact of OCT1 polymorphisms on the pharmacokinetics of other triptans as well. The uptake of sumatriptan could be inhibited through commonly used co-medication in patients with migraine. In conclusion, OCT1 is a high-capacity transporter of sumatriptan and polymorphisms causing OCT1 deficiency effects the uptake of sumatriptan. | de |
