dc.description.abstractgerDas Ziel dieser Arbeit bestand darin, herauszufinden, inwieweit eine Kombinationstherapie aus Strontiumranelat beziehungsweise Parathormon mit einer vertikalen Ganzkörpervibration bei 70 Hz einen positiven Effekt auf den osteoporotischen Lendenwirbelkörper der Ratte zeigt. Dafür erfolgte die Einteilung der 90 weiblichen Sprague-Dawley-Ratten in sieben Gruppen. Die erste Gruppe diente als Kontrollgruppe während alle anderen Gruppen ovariektomiert wurden: NON-OVX, OVX, OVX+VIB, SR, SR+VIB, PTH und PTH+VIB. Acht Wochen nach der Ovariektomie wurde eine Osteotomie der Tibiae durchgeführt. Danach erfolgte eine tägliche subkutane Applikation von 40 µg/kg KG Parathormon der Gruppen PTH und PTH+VIB. Zum gleichen Zeitpunkt erhielten die Gruppen SR und SR+VIB mit Strontiumranelat versetztes Futter in einer Dosis von 600-625 mg/kg KG. Die Gruppen OVX+VIB, SR+VIB und PTH+VIB wurden zwei Mal täglich für 15 Minuten mit einer vertikalen Ganzkörpervibration bei 70 Hz behandelt, die fünf Tage nach Osteotomie begann. Die Ratten wurden sechs Wochen nach Osteotomie getötet. Die Lendenwirbelkörper wurden präpariert und mit drei verschiedenen Methoden untersucht. Der zweite Lendenwirbelkörper wurde verascht und auf seine Zusammensetzung untersucht. Dabei wurden die Knochenmineraldichte, die organische und anorganische Masse sowie der Anteil an Calcium, Phosphat und Strontiumranelat bestimmt. Die Analyse der trabekulären und kortikalen Strukturen erfolgte durch eine Mikro-Computertomographie des dritten Lendenwirbelkörpers. Der vierte Lendenwirbelkörper wurde in einem biomechanischen Kompressionstest auf seine Dehnbarkeit und Elastizität geprüft. In allen drei Untersuchungen zeigten sich keine besseren Ergebnisse unter SR+VIB- beziehungsweise PTH+VIB-Therapie im Vergleich zu einer Einzeltherapie der jeweiligen Medikamente. Insgesamt erzielten die PTH- und PTH+VIB-Gruppen die besten Ergebnisse in Bezug auf die axiale Belastbarkeit im biomechanischen Kompressionstest. Parathormon wirkte sich effektiv auf die Festigkeit und Dehnbarkeit des Rattenlendenwirbelkörpers aus. Dabei führte es zu einem erhöhten Anteil an anorganischer Masse und zu einer Zunahme des Calciumgehalts. In der Mikro-Computertomographie ergab sich eine verringerte Trabekelanzahl, die durch eine Zunahme der Trabekeldicke kompensiert wurde. Die Strontiumranelat-Einzeltherapie erreichte in diesem Versuch eine signifikante Zunahme der Kortikalisdicke, wobei sich keine weiteren signifikanten Verbesserungen der Parameter ergaben. Damit zeigte sich die Strontiumranelatgabe einer Therapie mit Parathormon unterlegen. Die Ganzkörpervibration erzielte ebenfalls eine signifikante Verbesserung der Kortikalisbreite im Vergleich zu den ovariektomierten Ratten. Die mit Ganzkörpervibration behandelte Gruppe tendierte auch hinsichtlich BV/TV, Trabekelanzahl und trabekulärer Knotenpunkte zu höheren Werten im Vergleich zur OVX-Gruppe wobei sich keine signifikanten Unterschiede zeigten. Insgesamt führte eine Kombinationstherapie aus Strontiumranelat oder Parathormon mit einer vertikalen Ganzkörpervibration bei 70 Hz zu keiner signifikanten Verbesserung der erhobenen Knochenparameter. Letztendlich zeigte sich das Medikament Parathormon in der Therapie des osteoporotischen Rattenlendenwirbelkörpers einer Strontiumranelatgabe und einer Ganzkörpervibration überlegen.de
dc.description.abstractengThe aim of this work was to find out if a combination therapy of parathyroid hormone or strontium ranelate with a vertical vibration therapy at a frequency of 70 Hz affects the osteoporotic lumbar spine of the rat positively. Therefore, 90 female rats were divided into seven groups. The first group was used as the control group while all other groups were ovariectomized: NON-OVX, OVX, OVX+VIB, SR, SR+VIB, PTH and PTH+VIB. Eight weeks after ovariectomy the tibia was osteotomized. Afterwards, the groups PTH and PTH+VIB received a daily application of 40 µg/kg BW parathyroid hormone. At the same time the groups SR and SR+VIB received food containing strontium ranelate in a dosage of 600-625 mg/kg BW. The groups OVX+VIB, SR+VIB and PTH+VIB were treated with a vertical whole body vibration therapy at a frequency of 70 Hz for 15 minutes twice a day. The vibration treatments started five days after osteotomy. Six weeks after osteotomy the rats were killed. The lumbar vertebrae were dissected and analyzed by three different methods. The second lumbar vertebral body was ashed and evaluated regarding its bone mineral density, organic and inorganic mass and the proportion of calcium, phosphate and strontium ranelate. The trabecular and cortical structures were analyzed by a micro-computed tomography of the third vertebral body. A biomechanical compression test was used to examine the flexibility and elasticity of the fourth lumbar vertebra. All three examinations showed no better results in the SR+VIB and PTH+VIB groups in comparison to the single drugs therapy. Altogether, PTH and PTH+VIB treatment produced the best results in the biomechanical compression test concerning the axial capacity. Parathyroid hormone affects the stiffness and elasticity of the lumbar rat vertebra. Thereby, parathyroid hormone leads to a higher inorganic mass as well as to an increase of calcium content. The micro computed tomography showed a decrease of trabecular numbers, which was compensated by an increase of the trabecular thickness. The single strontium ranelate therapy reached a significant gain of the cortical thickness, but no other significant improvements were seen concerning the bone parameters. Therefore, strontium ranelate treatment shows to be inferior to the therapy with parathyroid hormone. Also, the whole body vibration achieved a significant enhancement of the cortical thickness in contrast to the ovariectomzed rats. In the vibrated group BV/TV, trabecular number and trabecular nodes were slightly improved in comparison to the ovariectomized group, although the differences were not significant. In conclusion, a combination therapy of strontium ranelate or parathyroid hormone with a vertical whole body vibration at a frequency of 70 Hz leads to no significant improvement of the investigated bone parameters. Finally, parathyroid hormone shows to be superior to strontium ranelate and whole body vibration in therapy of the osteoporotic rat lumbar spine.de
