dc.description.abstractgerBei der Subarachnoidalblutung (SAB) handelt es sich um eine folgenschwere Einblutung in den Subarachnoidalraum, deren Verlauf in 50 % der Fälle tödlich ist und rund ein Drittel der Überlebenden auf lebenslange Pflege angewiesen bleibt. Die häufigste Ursache der Blutung ist die Ruptur eines Aneurysmas (aneurysmatische SAB, aSAB). Das Outcome nach aSAB hängt neben der Größe des Aneurysmas und dem Ausmaß der Blutung auch maßgeblich von einer schnellen Diagnostik und Therapie sowie einer engmaschigen Nachsorge ab. Ziel dieser retrospektiven Studie war es herauszufinden, ob die Spezialisierung von Intensivstationen zu neurochirurgischen Intensivstationen (nICU) das Outcome von Patienten mit aSAB positiv beeinflusst. Analysiert wurde das Outcome von 755 Patienten, die zwischen 1990-2005 in einer nICU und zwischen 2005-2013 in einer generalisierten ICU (gICU) behandelt wurden. Die Behandlungsstrategien waren in beiden Zeiträumen identisch. Die Datenbanken beinhalteten unter anderem Daten wie das Alter und das Geschlecht, klinische Parameter (Glasgow Coma Scale (GCS), Hunt- und Hess- Grad, Fisher- Grad) und das Vorkommen von Vasospasmen und neurologischen Defiziten nach der Therapie. Das Outcome wurde anhand der Glasgow Outcome Scale (GOS) beurteilt. Mit Hilfe einer multiplen Regressionsanalyse wurden der GCS-Wert, der Fisher- Grad, und der Hunt- und Hess Grad als signifikante Einflüsse auf das Outcome der Patienten identifiziert. Der Vergleich der beiden Patientenkollektive ergab ein besseres Outcome bei den Patienten, die in der nICU behandelt wurden. Diese Studie lässt darauf schließen, dass sich eine neurochirurgische Spezialisierung von Intensivstationen positiv auf das Outcome von Patienten mit aSAB auswirkt.de
dc.description.abstractengThe subarachnoid hemorrhage (SAH) is a devastating bleeding into the subarachnoid space leading to death in 50 % of all cases. One third of the survivors are dependend on lifelong care. The main cause for SAH is the rupture of an aneursym (aSAH, aneurysmal SAH). Besides the size of the aneurysm and the bleeding, a prompt diagnosis and therapy as well as post-operative care are the main factors that influence the outcome. The aim of this study was to find out if the specialisation of the intensive care unit (ICU) the patients are treated in, has an impact on their outcome. We analysed the outcome of 755 patients with aSAH treated in either a nICU (between 1990-2005) or a general ICU (gICU, between 2005-2013) with identical treatment strategies. The database contained information about the age and sex, clinical data (Glasgow Coma Scale(GOS), Hunt and Hess Score(H&H)), the appearance of vasospams and delayed neurological complications. The outcome was analysed by the basis of the Glasgow Outcome Scale (GOS). The multivariate analysis showed a better outcome in the group of the patients treated in the nICU. Factors with a significant influence on the outcome were the initial GCS, the Fisher Score and the H&H. This study comes to the conclusion, that a neurosurgical specialisation of an ICU has a positve impact on the outcome of patients with aSAH.de
