Ebstein's anomaly is a rare congenital heart disease with potential affection of not only the right but also the left heart function that may play a role in heart failure development. This thesis aimed to assess quantitative atrial and ventricular function in Ebstein's anomaly with cardiovascular magnetic resonance feature tracking and to correlate changes in biatrial and biventricular performance with the severity of disease and clinical parameters of heart failure (BNP and NYHA class). The Feature Tracking software calculates the myocardial deformation from cardiovascular magnetic resonance imaging and allows the quantification of the contractile function of the heart. In this study the atrial and ventricular deformation parameters and additionally the volumetric measurements from 30 patients with Ebstein´s anomaly and 20 healthy controls were studied at 1.5 Tesla. The patients with Ebstein’s anomaly showed a significantly impaired right atrial and ventricular performance which correlated with markers of heart failure. Furthermore, their left atrial function was also significantly impaired compared to healthy controls with left atrial contractile function correlating with NYHA class. These novel quantitative performance parameters from cardiovascular magnetic resonance may represent early markers of cardiac deterioration of potential value to improve the clinical management of patients with Ebstein’s anomaly, for example for the decision making for intervention or operation.

Die Ebstein’sche Anomalie ist ein seltener angeborener Herzfehler, welcher sowohl die Rechtsherz- als auch die Linksherzfunktion einschränken kann und so zu einer Herzinsuffizienz führen kann. Diese Arbeit beschäftigt sich mit aus der kardiovaskulären Magnetresonanztomographie mittels Feature Tracking gewonnenen quantitativen atrialen und ventrikulären Funktion der Patienten mit Ebstein’scher Anomalie und korreliert diese Veränderungen in der Vorhof- und Ventrikelleistung mit dem Schweregrad der Erkrankung und klinischen Herzinsuffizienzparametern (BNP und NYHA-Klassifikation). Die Feature Tracking-Software berechnet die myokardiale Deformation aus der kardiovaskulären MRT-Bildgebung und erlaubt damit die quantitative Beurteilung der kontraktilen Funktion des Herzens. In dieser Studie wurden die atrialen und ventrikulären Deformationsparameter sowie zusätzlich die kardiale Volumetrie von 30 Patienten mit Ebstein’scher Anomalie sowie 20 gesunden Kontrollpersonen in einem 1,5 Tesla MRT untersucht. Die Patienten mit Ebstein’scher Anomalie zeigten hierbei eine signifikant verringerte Rechtsherzfunktion, welche mit den klinischen Herzinsuffizienzparametern korrelierte. Außerdem zeigte sich eine im Vergleich zur gesunden Kontrollgruppe signifikant eingeschränkte linksatriale Funktion mit einer Korrelation der Kontraktionsfunktion mit der NYHA-Klassifikation. Diese neuen Deformationsparameter zeigen möglicherweise eine Einschränkung der kardialen Funktion früher an als bisher gebräuchliche Parameter und könnten damit in der Zukunft eine Rolle im klinischen Management der Ebstein’schen Anomalie spielen, zum Beispiel bei der Entscheidung zu einer Korrektur-OP.
