dc.description.abstractengIn medical terminology the term „transition“ denotes the purposeful and planned movement of adolescents with chronical diseases from a child-centred to an adult-centred health-care-system. Many clinical models have been developed yet to improve the medical care in different pediatric subdisciplines during transition. At present children and youths with chronical neurological diseases or motoric and mental disabilities are treated at SPZs (Sozialpädiatrische Zentren) by the age of 18. The treatment would only be continued on exceptional occasions. This study explores the link between the complexity and severity of the chronical neurological disease of patients from the SPZ Göttingen and the success of the transition. Therefor the patients’ satisfaction with the current adult-centred medical health care was measured. Social and medical data and the satisfaction of the patients were recorded by questionnaires and medical records of the SPZ. The study included 50 patients. The participants were classified into two groups. The first group included patients with complex neurological diseases or disabilities, the second group included patients with less pronounced impairments. The results show that patients from group 1 are significantly less satisfied with the treatment in the adult-centred health care system than patients with less pronounced impairments. With respect to the approval of MZEBs (Medizinische Zentren für Erwachsene mit Behinderungen) in July 2015, in which adults with complex and severe diseases or disabilities can be treated among others, the results of this study emphasise the need of these institutions.de
