Abstract

English

Introduction: The main reason for the impending critical shortage of GPs in rural areas is a shortage of young doctors who decide to become GPs, and to practise in rural areas. The Biographical experiences of GPs who have themselves only recently established their practices may be helpful in staunching this development. Study Question: Which experiences, especially during their training, induce young doctors to establish themselves in rural areas? Which factors are conducive and which are inhibitive influences on the decision-making process? Methods: In the context of a qualitative research study we conducted 24 narrative one-on-one interviews with GPs who had established themselves in rural areas of Lower Saxony within the last five years. Selection of the interviewees was based on the priciples of “maximum variation” and “theoretic sampling”. The sequence of interviews was a direct result of parallel evaluated interviews and thus generated preliminary hypotheses. These were then analyzed together with similar or contrasting cases. Data analysis of the interview transcriptions was carried out in accordance with the grounded theory method. Results: A keen desire for quality of life was articulated in almost all of the interviews. The motives for the choice of consultant training revealed an area of conflict: negative experiences during training especially during hospital assignments acted as an deterent. Positive factors, especially an increase in individual quality of life due to the reduction of work related stress factors, increased prosperity and better work-life balance, made the GP practice appear more attractive. The future GPs were impeded even before beginning their studies by their “Abitur” results.
