Spine injuries only make a small part of all skeletal injuries of the human body but they are often responsible for chronical pain, missing the ability to recover into professional life or limitation in life in general. In this retrospective study all treated thoracic and lumbar spine fractures between 2008 and 2013 were analyzed in regard to the category of fracture, the way of treatment and the radiological outcome. For classification of the fracture type the Magerl classification (1994) has been used. In literature the Magerl groups A1 and A2 (compression type) are known as stable fractures which are treated conservatively. B- (flexion type) and C- (rotation type) fractures are highly instable fractures and must be stabilized surgically either by posterior pedicel fixation with an internal fixator or combined with anterior decompression and fixation. The treatment strategy for burst fractures without neurological deficits (A3 category in Magerl classification) remains controversial. In the retrospective analysis the kyphotic angle, the sagittal plane kyphosis and the ratio of heights of the anterior and posterior vertebral body were measured using radiological images as CT and X-Ray before and after surgical intervention and in a 2 weeks, 4 weeks and 3 months follow up until removal of the inernal fixator. The collective contains 475 Patients with 513 fractures of the thoracic and lumbar spine. Most of the fractures were localized in the thoarcolumbar region. It was observed that no significant correction is gained by treating A1 and A2 fractures surgically instead of consevatively. Burst fractures without neurological deficits (A3) are better stabilized by combined surgical techniques. Furthermore the results show a high loss of correction after the removal of the osteosynthesis material in B- and C- fractures. It can be concluded that A3- Fractures should in any case be treated surgically and more often in combination with anterior decompression and fixation while in instable B- and C- fractures the fixator should be remained in situ for a longer time to keep the correction in a solid level.

Aufgrund der Tatsache, dass die Wahl der Therapie bei Wirbelfrakturen der Klasse A3 nach
Magerl et al. (1994) weiterhin kontrovers diskutiert wird, wurden in dieser Arbeit die
Patientenfälle der Abteilung Orthopädie und Unfallchirurgie der Universitätsmedizin
Göttingen betrachtet, die im Zeitraum von 2008 bis 2013 eine Wirbelfraktur im Bereich der
Brust- und Lendenwirbelsäule erlitten. Das gesamte Kollektiv umfasst 475 Patienten, von denen 266 männlich
und 209 weiblich sind. Es wurden objektive Daten wie GDW, Kyphosewinkel und
Wirbelkörperhöhenminderung zu fest definierten Zeitpunkten erhoben. Anhand dieser
Daten konnte der Verlauf der Stabilität und sowohl der Korrekturgewinn durch die Therapie
als auch der Korrekturverlust während der Nachsorge für die jeweiligen Therapiemethoden
erfasst werden. Die Therapiemethoden, die hier verglichen werden, sind die konservative
Therapie, die Kyphoplastie, die dorsale und die kombiniert dorsoventrale Stabilisierung. Die in der radiologischen Verlaufskontrolle erhobenen Parameter zeigen, dass bei Frakturen
der Klassen A1 und A2, die als stabil gelten, die konservative Therapie zu einem guten
Ergebnis führt. Bei A3- Frakturen, bei denen in der internationalen Literatur starke Unstimmigkeit darüber besteht, ob diese Frakturen als stabil oder instabil gelten und wie solche
Frakturen im allgemeinen zu behandeln sind, bringt die operative Vorgehensweise klare
Vorteile. Zudem können bei kombiniert dorsoventraler Operation noch stabilere Ergebnisse
erzielt werden. Für die Rotationsfrakturen stellt sich heraus, dass nach der Entfernung des
eingebrachten Osteosynthesematerials ein größerer Korrekturverlust eintritt. Abschließend lässt sich sagen, dass bei isolierten Berstungsfrakturen ohne Beteiligung des
Bandapparates die Indikation für die kombiniert dorsoventrale Operation häufiger und
frühzeitiger gestellt werden sollte. Außerdem sollte beachtet werden, dass bei instabilen
Frakturen die Verlängerung der Tragezeit des Fixateurs einen positiven Einfluss auf den
Korrekturerhalt hat.
