dc.description.abstractengSudden cardiac death (SCD) due to caradiac arrhythmias is a major complication in patients with cardiac disease. However risc stratification is improvable as life-threatening cardiac arrhythmias do not occur in many patients carrying an intracardiac cardioverter-defibrillator (ICD). This dissertation aims an improvement of risk analysis of SCD. In the study thirty-seven single nucleotide polymorphisms associated with life-threatening cardiac arrhythmias are identified. Furthermore, twohundred-fifty patients carrying an ICD were genotyped. In these patients parameters of cardiac electrophysiology and other cardiac parameters in relation to life-threatening cardiac arrhythmias were assessed. In the following, when resigning the statistical means of multiple testing, correlations between genotype and assessed parameters occur. Polymorphisms in CSQ2 -a gene involved in homeostasis of calcium in the cardiomyocyte- were associated with parameters of electrophysiology that implicate an increased risk of SCD. Pathological characteristics in T-wave alternans can be an expression of deranged repolarisation of the cardiomyocyte as a result of a disequilibrium in calcium balance. However, when applying multiple testing no significant associations occur. This thesis gives poor evidence of significant relations between the investigated genotypes and risk of SCD. However, it has to be investigated in a prospective study if certain genotypes show relations to life-threatening cardiac arrhythmias which are poorly predicted by the used parameters of electrophysiology. Presumably future studies will find other associations between polymorphisms and SCD which will improve the means of risk stratification.de
