PURPOSE: To investigate how often lesions with unknown biological potential (B3-lesions) occur in a biopsy collective and to evaluate the results of follow-up and open excision. METHOD: Retrospective study from May 2003 - April 2013. 145 women with histological secured B3-lesion and previous radiological imaging. Evaluation of its imaging presentation, further procedure (excising versus not excising) and the occurance of malignant lesions in follow-up. RESULTS: The amount of patients with lesions of unknown biological potential (B3-lesions) in the total biopsy collective was 145/1935 (7,5 %). 60 % of all B3-lesions underwent open excision, of which 18 % were afterwards diagnosed as malignant. The highest rate of malignant findings were in ADH und FEA (40 %), followed by LIN (30 %) and papillary lesions (11 %). Additionally we found two carcinomas in the follow-up of 12 not operated women with LIN. All malignant findings were small (< 10 mm) and prognostically favorable. CONCLUSION: After detection of LIN and ductal atypia (ADH, FEA) via percutanous breast biopsy a full excision and in any case a close follow-up is recommended, since malignancy is present or has to be suspected in 30 - 40 %. The rate of carcinomas related to papillary lesions and radial scars on the other hand is respectively low with about 10 %.

Ziel: Untersuchung mit welcher Häufigkeit B3-Läsionen in einem Biopsie-Kollektiv auftreten und zu welchen Ergebnissen sowohl die Beobachtung im Verlauf als auch die operative Befundabklärung führen. Methode: Retrospektive Studie von Mai 2003 bis April 2013. 145 Frauen mit histologisch gesicherter B3-Läsion und vorausgegangener Bildgebung. Auswertung bezüglich des bildgebenden Erscheinungsbildes, des gewählten weiteren Vorgehens (operiert versus nicht-operiert) und des Auftretens von malignen Läsionen im Verlauf. Ergebnisse: Der Anteil der histologischen Befunde mit unklarem biologischen Potential (B3-Läsionen) betrug im gesamten Biopsie-Kollektiv 145/1935 (7,5 %). 60 % aller B3-Läsionen wurden mittels offener Biopsie abgeklärt. Histologisch ergab sich in 18 % postoperativ ein bösartiger Befund. Diese Rate war am höchsten nach Exzision von ADH und FEA (40 %) gefolgt von LIN (30 %) und papillären Läsionen (11 %). Zusätzlich fanden sich im Follow-up von 12 nicht-operierten Frauen mit LIN zwei Karzinome. Alle gefundenen Tumore waren klein (< 10 mm) und prognostisch günstig. Schlussfolgerung: Bei perkutan-bioptischer Sicherung von LIN und atypischen duktalen Veränderungen (ADH, FEA) ist eine operative Befundentfernung und in jedem Fall ein sehr konsequentes Follow-up ratsam, da in 30 - 40 % der Fälle Malignität vorliegt oder droht. Im Gegensatz hierzu liegt die Karzinomquote bei papillären Läsionen und radiären Narben im eher überschaubaren Bereich um 10 %.
