In medicine, again and again there are situations in which patients or their relatives request therapy that is not (or no longer) indicated or reasonable from a medical point of view. This applies in particular to the area of intensive care medicine. In current literature, such treatment scenarios and the concept of medical indication are under discussion, as is the concept of medical futility used in this context in the US. Based on an analysis of literature, the present study examines statements and recommendations by medical societies and professional organizations in Germany and the USA on the subject of "therapy limitation due to a lack of indication", focusing attention on the proposals for an ethically appropriate procedure in conflict situations. Its aim was to determine the similarities and differences and to examine the extent to which elements of the US futility debate can be transferred to Germany.

In der Medizin kommt es immer wieder zu Situationen, in denen Patienten oder ihre Angehörigen Therapien einfordern, die aus ärztlicher Sicht nicht (mehr) indiziert bzw. sinnvoll sind. Dies trifft insbesondere auf den intensivmedizinischen Bereich zu. In der gegenwärtigen Literatur werden derlei Behandlungsszenarien und der Begriff der medizinischen Indikation kontrovers diskutiert, ebenso wie der im US-amerikanischen Raum in diesem Kontext verwendete Begriff der medical futility. Im Rahmen einer Literaturanalyse wurden in der vorliegenden Arbeit Stellungnahmen und Empfehlungen medizinischer Fachgesellschaften und Standesorganisationen aus Deutschland und den USA zum Thema „Therapiebegrenzung aufgrund fehlender Indikation“ untersucht und die enthaltenen Vorschläge für ein ethisch angemessenes Vorgehen in Konfliktsituationen herausgearbeitet. Ziel war es, Gemeinsamkeiten und Unterschiede zu ermitteln und zu überprüfen, inwiefern sich Elemente der US-amerikanischen Futility-Debatte auf Deutschland übertragen lassen.
