dc.description.abstractger | Menschen mit Schizophrenie weisen im Vergleich zu einer gesunden Kontrollgruppe eine
beeinträchtigte Neuroplastizität auf. So konnte in vielen Untersuchungen mit Hilfe der
TMS bei schizophren Erkrankten eine verringerte motorkortikale Inhibition gezeigt
werden. Für Sportinterventionen, insbesondere aerobes Ausdauertraining, ergaben sich in
mehreren Studien positive Effekte auf die Kognition und Neuroplastizität bei Gesunden
und bei Menschen mit Schizophrenie.
In meiner Dissertation wurde nun erstmals untersucht, ob sich die motorkortikale
Exzitabilität über dem linken und rechten primärmotorischen Kortex, gemessen mit
Paradigmen der diagnostischen TMS, durch eine dreimonatige plastizitätsinduzierende
Sportintervention bei Menschen mit einer Schizophrenie und einer gesunden
Teilnehmergruppe signifikant verändert und ob sich zwischen den Gruppen hierbei
signifikante Unterschiede ergeben.
Die Probanden für diese Untersuchung wurden im Rahmen der Hauptstudie „Exercise-II“,
welche von 2010 bis 2013 an der Klinik für Psychiatrie und Psychotherapie der
Universitätsmedizin Göttingen durchgeführt wurde, rekrutiert. Dabei wurden für meine
Doktorarbeit Menschen mit der Diagnose einer Schizophrenie, welche stationär oder
ambulant in der Klinik für Psychiatrie und Psychotherapie der Universitätsmedizin
Göttingen behandelt wurden, und gesunde Teilnehmer aus den Ergometriegruppen der
Hauptstudie (nicht aus der Tischfußballgruppe), rekrutiert. In meine Studie konnten 17
Menschen mit Schizophrenie und 16 gematchte Kontrollpersonen eingeschlossen werden,
die ein dreimonatiges aerobes Ausdauertraining (Fahrradergometrie über 30 Minuten,
dreimal pro Woche) und nach sechs Wochen ein zusätzliches computerbasiertes kognitives
Training (CogPack über 30 Minuten, zweimal pro Woche) erhielten. Die Erregbarkeit der
motorischen Kortices wurde vor (Visite 1, Tag 0) und nach diesem drei Monate
andauernden Programm (Visite 3, Tag 84) mit Hilfe der TMS gemessen und dabei die
Intensität, mit der ein motorisch evoziertes Potential (MEP) von 1 mV hervorgerufen wird
(S1 mV), die motorische Reizschwelle in Ruhe (RMT), die kontralaterale kortikale
Innervationsstille (CSP) bei einer Reizintensität von 120 % und 150 % der RMT, die Shortlatency
Interval Intracortical Inhibition (SICI bei 3 ms Interstimulusintervall) und die
intrakortikale Fazilitation (ICF bei 7 ms und 15 ms Interstimulusintervall) bestimmt. Je
nach Paradigma und Hemisphäre konnten die Messwerte nach drei Monaten bei 7-15
Menschen mit Schizophrenie und 10-12 gesunden Kontrollen erhoben werden.
Insgesamt ergaben sich keine signifikanten Unterschiede der Effekte zwischen den
Gruppen (Zeit x Gruppe Interaktion in der Messwiederholungs-Varianzanalysen), aber es
zeigte sich in beiden Gruppen ein Anstieg der S1 mV (F(1,17) = 5,334, p = 0,034) und der
CSP 150 % (F(1,19) = 8,795, p = 0,008) über der linken Hemisphäre.
Damit konnte erstmals an einer kleinen Stichprobe im Sinne einer longitudinalen Proof-ofconcept-
Studie nachgewiesen werden, dass aerober Ausdauersport (Ergometertraining)
über drei Monate zu einer verbesserten motorkortikalen Inhibition nicht in allen (wie
beispielsweise der SICI), aber in einigen Paradigmen (S1 mV, CSP) führen kann.
Diese Hinweise müssen allerdings erst in einer weiteren Studie mit höherer Probandenzahl
und ausschließlicher Sportintervention (ohne kognitives Training) repliziert werden, um
einen anzunehmenden Effekt auf inhibitorische, GABAerge, motorkortikale Netzwerke im
Sinne einer durch Sport verbesserten Neuroplastizität bei Gesunden und schizophren
Erkrankten annehmen zu können.
Als Ausblick ist hervorzuheben, dass nicht nur bei Gesunden, sondern auch bei Menschen
mit einer Schizophrenie und inhibitorischen Defiziten in diesem Bereich, neuroplastische
Veränderungen durch Sport induziert werden und therapeutisch weiter genutzt werden
