Aim: The aim of the present study was to investigate the influence of tooth brushing training using the differential learning approach on plaque and gingivitis reduction in primary school children and to compare it with the method of conventional instruction and demonstration of a tooth brushing technique and habitual tooth brushing. Material and method: In the study, 54 schoolchildren between the ages of six and nine were divided equally into three treatment groups of 18 children each. The three groups were then subjected to tooth brushing training (3 weeks, 5 working days each, daily: approx. 3 minutes): Group A (differential learning of tooth brushing), Group B (conventional instruction and demonstration of tooth brushing), Group C (habitual tooth brushing technique). The differential learning approach (group A) consisted of 15 different movement exercises (1/day), while group B was instructed in the KAI method, which is based on the principle of repetition and correction. To control oral hygiene, the Turesky-modified Quigley-Hein index (T-QHI) and the papillary bleeding index (PBI) were used at the start of the study (t0), after 21 days (t1), 42 days (t2) and measured at 63 days (t3). Statistical analysis was performed using ANOVA and logistic regression (p<0.025). Results: At time points t1 to t3, differential learning led to better results than conventional instruction and demonstration and the habitual brushing technique. Compared to t0, differential learning resulted in significantly improved gingivitis and plaque reduction at all time points. No significant reduction in PBI could be observed in the other groups. The T-QHI was significantly reduced by habitual brushing only at time t1 and by conventional instruction and demonstration at time t1 and t2. The differential learning method had a better impact on the molars than on the anteriors. Conclusion: The differential learning method leads to significantly better oral hygiene than habitual brushing and brushing after conventional instruction and demonstration.de
