dc.description.abstractger | Klinische Forschung ist die Grundlage für den Fortschritt in der Medizin. Ihr wichtigstes Instrument sind klinische Studien, in denen an und mit Teilnehmenden geprüft wird, wie wirksam, verträglich und sicher neue Medikamente und Behandlungsmöglichkeiten sind. Bei vielen wissenschaftlichen Fragestellungen ist es sinnvoll, in standortübergreifenden Kooperationen gemeinsam zu forschen. Dies stellt hohe Anforderungen an die Organisation, die Kommunikation und die Arbeitsprozesse, aber auch an die eingesetzten IT-Systeme. So ist die standortübergreifende Verwaltung von Studienteilnehmenden mit einem Probandenmanagementsystem eine große Herausforderung. Besonders die zentrale Verarbeitung der sensiblen Daten der Studienteilnehmenden von unterschiedlichen Studienstandorten beeinflusst die Datenschutz- und IT-Sicherheitsanforderungen. Dabei ist auch das Zusammenspiel mit weiteren IT-Systemen relevant, weshalb die Aufgaben und Funktionen des Probandenmanagementsystems klar benannt und dessen Rolle in einer Forschungs-IT-Infrastruktur geklärt sein muss. In dieser Dissertation wurde der Ausbau des 2015 am Institut für Medizinische Informatik der Universitätsmedizin Göttingen entwickelten Probandenmanagementsystems für die Nutzung in standortübergreifenden Kooperationen in der klinischen Forschung untersucht.
In zwei systematischen Literaturrecherchen wurden Literaturquellen mit dem Ziel der Begriffs- und Funktionsbestimmung des Probandenmanagementsystems und der Einordnung des Systems in einer Forschungs-IT-Infrastruktur untersucht. Vier Hauptprozesse der Probandenverwaltung wurden identifiziert: Rekrutierung (1), Erfassung und Verwaltung der Einwilligungen (2), Pseudonymisierung (3) und Verwaltung administrativer Daten (4) der Studienteilnehmenden. Die Prozesse können durch IT-Systeme unterstützt werden, die untereinander über Schnittstellen interagieren und so in einer Forschungs-IT-Infrastruktur zusammenarbeiten. Dabei ist die Zusammensetzung der IT-Systeme abhängig von den Anforderungen des Forschungsprojektes. Das bestehende Göttinger Probandenmanagementsystem vereint die IT-Unterstützung der genannten Hauptprozesse ausgenommen der Rekrutierung und übernimmt damit zentrale Aufgaben und Funktionen in einer Forschungs-IT-Infrastruktur. Zur Erarbeitung eines Konzeptes zum Ausbau des Probandenmanagementsystems wurden die Anforderung an ein in standortübergreifenden Kooperationen nutzbaren Systems identifiziert. Das Konzept gliedert sich angelehnt an die identifizierten Anforderungen in die Teile: Datentrennung bei der Probandenverwaltung (1), vernetze Probandenverwaltung (2), Zugangs- und Zugriffskontrolle (3) und Datenschutzkonzept (4). Das Konzept sieht die Anwendung von Grundprinzipien datenschutzgerechter Lösungen wie Datensparsamkeit und informationelle Gewaltenteilung vor. Um eine strikte Datentrennung bei der Probandenverwaltung zu gewährleisten, dürfen nur ausgewählte Pseudonyme in der Datenbank des Probandenmanagementsystems gespeichert werden. Das Anlegen neuer Pseudonyme sowie die Abfrage bestehender Pseudonyme wird über eine Schnittstelle zum Pseudonymisierungsdienst realisiert. Der sichere Zugriff auf Daten und Funktionen wird durch technische Datenschutzmaßnahmen zur Zugangs- und Zugriffskontrolle gewährleistet. Vorbereitend auf ein konkretes Forschungsprojekt wurden alle für ein Datenschutzkonzept relevanten Punkte zusammengetragen. Die konzeptionierten Maßnahmen wurden prototypisch implementiert, um so die Machbarkeit des Konzeptes zu bestätigen.
Standortübergreifende Kooperationen in der klinischen Forschung stellen hohe Anforderungen an ein Probandenmanagementsystem. Werden Datenschutz- und IT-Sicherheitsanforderungen nicht berücksichtigt und getroffene Maßnahmen in einem Datenschutzkonzept nicht ausreichend begründet, können nützliche IT-Systeme in der klinischen Forschung nicht eingesetzt werden. Das Konzept für die standortübergreifende Nutzung des Probandenmanagementsystems setzt an diesem Punkt an, berücksichtigt etablierte Datenschutzmodelle und setzt aktuelle IT-Sicherheitsempfehlungen um. So kann das Probandenmanagementsystem zur klinischen Forschung beitragen und Prozesse der Probandenverwaltung unterstützen. Damit werden Ressourcen in klinischen Forschungsprojekten effizient genutzt und neue Therapien können schneller in die Versorgung gelangen. | de |
dc.description.abstracteng | Clinical research is key for medical progress. Clinical trials are the most important instrument of clinical research, which are used to test efficacy and safety of new drugs and new forms of treatment on human participants. Joint research in multi-site collaborations are of particular importance to enable significant research results. Multi-site research imposes high demands on organization, communication and work processes, but also on the IT systems used. The multi-site administration of study participants with a Study Participant Management System is a major challenge. In particular, the central processing of sensitive data of study participants from different study sites determines the data protection and data security requirements. Further, the interaction with other IT systems is highly relevant, which is why the tasks and functions of the Study Participant Management System must be clearly specified and its role in a research IT infrastructure clarified. In this dissertation, the extension of the Study Participant Management System developed in 2015 at the Department of Medical Informatics, University Medical Center Göttingen, for the use in multi-site collaborations in clinical research, was investigated.
Two systematic literature reviews were conducted with the goal to define the term Study Participant Management System and to specify its functions and how the system integrates into a research IT infrastructure. Four main participant-related management processes were identified: recruitment (1), obtaining and managing informed consent (2), pseudonymization (3), and management of administrative data (4) of study participants. These processes can be supported by IT systems that interact with each other via interfaces and thus collaborate in a research IT infrastructure. The composition of the IT systems depends on the requirements of the research project. The Study Participant Management System from Göttingen combines the computer-supported management for the main processes, except for recruitment, and thus undertakes central tasks and functions in a research IT infrastructure. To develop a concept for the extension of the Study Participant Management System, the requirements for a system that can be used in multi-site collaborations were identified. Based on the identified requirements, the concept was divided into the following sections: data separation in participant management (1), interconnected participant management (2), access control (3) and data protection concept (4). The concept for the system extension provides the implementation of basic principles of data protection such as privacy by design and informational separation of powers. To ensure strict data separation in participant management, only selected pseudonyms may be stored in the database of the Study Participant Management System. Requests of new pseudonyms as well as the queries of existing pseudonyms are realized via an interface to the pseudonymization service. Secure access to data and functions is ensured by technical data protection measures for access control. In preparation for tangible research projects, all relevant aspects for a data protection concept were composed. The conceptualized measures were implemented prototypically to confirm the feasibility of the concept.
Multi-site collaborations in clinical research place high demands on a Study Participant Management System. If data protection and IT security requirements are not taken into account and measures taken are not sufficiently justified in a data protection concept, useful IT systems cannot be used in clinical research. The concept for the multi-site use of the Study Participant Management System addresses this issue, takes into account established data protection models and implements current IT security recommendations. Thereby, the Study Participant Management System can contribute to clinical research and support trial processes by managing participants’ data. As a result, resources in clinical research projects are used efficiently and new therapies can be introduced into medical care faster. | de |
