dc.description.abstractgerIn der vorliegenden Dissertation wird der Frage nachgegangen, ob eine mehrmonatige transkutane Vagus-Nerv-Stimulation (t-VNS) bei Patienten mit einer chronischen Schizophrenie eine Veränderung der motorkortikalen Erregbarkeit zur Folge hat. Meine Studie ist dabei die erste Arbeit, welche neurophysiologische Effekte von t-VNS an Patienten mit einer Schizophrenie untersucht (hier mittels transkranieller Magnetstimulation (TMS)). Es wurden insgesamt zehn Patienten mit einer Schizophrenie über mehrere Monate kontinuierlich mit t-VNS (Verum- oder Scheinstimulation) im Rahmen einer bizentrischen Studie zur Wirksamkeit der t-VNS auf die Symptome der Schizophrenie behandelt. Um den Effekt der t-VNS Therapie auf die motorkortikale Erregbarkeit zu bestimmen, führte ich im Verlauf der t-VNS (vor Beginn und zwölf Wochen später) mehrere Untersuchungen mittels verschiedenen TMS-Einzel- und Doppelimpulsmessungen (Bestimmung der motorischen Schwelle in Ruhe, 1mV-Intensität, intrakortikale Fazilitation mit 7- und 15-ms-Interstimulusintervall und kortikale Inhibition mit einem Interstimulusintervall von 3 ms) durch. Das Hauptergebnis meiner Untersuchung ist eine Reduktion der intrakortikalen Fazilitation (ICF, Interstimulusintervall 15 ms) über dem linken primären Kortex (entspricht der stimulierten Hemisphäre) in der Verum-Gruppe verglichen mit der Placebo-Gruppe. Dieser Effekt kann unter Berücksichtigung vorhandener Literatur, die konsistent eine abnorme Übererregbarkeit bei Patienten mit einer Schizophrenie berichtet, im Sinne einer Normalisierung verstanden werden. In der Sham-Gruppe bleibt die ICF hingegen konstant und zeigt keine Reduktion in der Erregbarkeit. Zusammenfassend stehen die wissenschaftlichen Forschungen zur Wirkung der t- VNS bei Patienten mit einer Schizophrenie noch am Anfang und meine Ergebnisse der möglichen neuromodulatorischen Wirkung der t-VNS müssen an einem größeren Patientenkollektiv in weiteren Studien repliziert werden.de
dc.description.abstractengIn this doctoral thesis, I explore if several months of transcutaneous vagal nerve stimulation will induce a change in motorcortical excitability in patients with chronic schizophrenia. This study is the first that investigates the neurophysiologic effect of t-VNS on patients with schizophrenia (by using transcranial magnetic stimulation, TMS). Embedded in a clinical proof-of-concept bicentric study investigating the efficacy of t-VNS on clinical symptoms of schizophrenia, a total of ten patients with schizophrenia has been treated continuously with t-VNS (verum or sham stimulation) over several months. In order to determine the effect of t-VNS therapy on motorcortical excitability, I performed (at baseline and after twelve weeks) several assessments using different single- and double-pulse TMS-techniques (measuring resting motor threshold, 1mV MEP intensity, intracortical facilitation using a 7 and 15 ms inter-stimulus interval and cortical inhibition using an interstimulus interval of 3 ms). The main result of my study is a reduction in intracortical facilitation (ICF, interstimulus interval 15 ms) over the left primary cortex (corresponding to the stimulated hemisphere) in the verum group compared to the sham group. This effect can be understood as normalization, taking into account existing literature, which consistently reports abnormal over-excitability in patients with schizophrenia (Radhu et al., 2013, Bunse et al. 2014). In the sham group, the ICF remained constant and showed no reduction in excitability. In summary, the scientific research on the efficacy of t-VNS in patients with schizophrenia is still at the beginning and my results of a possible neuromodulatoric effect of t-VNS should be replicated in further studies and studies with larger sample sizes.de
