dc.description.abstractgerDie Tumorresektion mit anschließender freier Lappenplastik ist eine gängige Therapie nach großflächigen Tumorerkrankungen in der Mund-Kiefer-Gesichtschirurgie. Eine der häufigsten schwerwiegenden Komplikationen bei einer Lappentransplantation ist eine insuffiziente Durchblutung des transplantierten Gewebes und daraus resultierend eine notwendige Revisionsoperation oder sogar ein Versagen des Transplantats. Bei großen und langwierigen Operationen, wie zum Beispiel einer Lappentransplantation, ist die perioperative Gabe von Katecholaminen eine gängige Praxis zur Kreislaufstabilisierung. Da Katecholamine auch gefäßverengend wirken, wurde lange vermutet, dass die Gabe von Katecholaminen den Erfolg einer Lappenplastik gefährden könnte. Deswegen ist das Ziel dieser Untersuchung, die Auswirkung der Gabe von Katecholaminen bzw. hier speziell von Noradrenalin auf den Erfolg von Lappenplastiken im Mund-Kiefer-Gesichtsbereich zu überprüfen. Dazu wurden in dieser retrospektiven Studie Daten von 103 Patient*innen der Mund-Kiefer-Gesichtschirurgie ausgewertet, die zwischen 2016 und 2019 nach einer Tumorresektion eine plastische Rekonstruktion mittels freier, mikrovaskulärer Lappenplastik erhielten. Das Hauptergebnis dieser Studie ist, dass kein Zusammenhang zwischen der intraoperativen Gabe von Katecholaminen auf das Outcome des freien Gewebetransplantates beobachtet wurde. Tatsächlich konnte statistisch für keinen der untersuchten Parameter ein Einfluss auf den Erfolg der Lappenplastik, der über die Outcomes „flap failure < 24 h“ und „Transplantatkomplikationen > 24 h“ erfasst wurde, nachgewiesen werden. Die Ergebnisse dieser Studie zeigen übereinstimmend mit der aktuellen medizinischen Forschung, dass die intraoperative Gabe von Katecholaminen als wichtiges anästhesiologisches Instrument zur Kreislaufstabilisierung auch bei freien Lappentransplantaten nach Tumorresektion in der Mund-, Kiefer- und Gesichtschirurgie eingesetzt werden kann.de
dc.description.abstractengTumor resection followed by a free flap is a common therapy for oral and maxillofacial tumors. One of the most common serious complications of flap transplantation is insufficient perfusion of the transplanted tissue, which might result in the need for revision surgery or might even lead to failure of the transplant. In major and lengthy surgeries such as flap transplantation, perioperative administration of catecholamines is a common practice to stabilize the patients’ circulation. Since catecholamines also have a vasoconstrictive effect, it has long been suspected that the administration of catecholamines could negatively impact the success of a flap. Therefore, the goal of this study is to examine the effect of the administration of catecholamines, specifically norepinephrine, on the success of oral and maxillofacial free flaps. In this retrospective study, data from 103 patients between 2016 and 2019 undergoing oral and maxillofacial plastic reconstruction with a free, microvascular flap after tumor resection were evaluated. The main result of this study is that no association between the intraoperative administration of catecholamines with the outcome of the free tissue graft was observed. In fact, no influence on the success of the flap, which was recorded via the outcomes "flap failure < 24 h" and "graft complications > 24 h", could be statistically supported for any of the examined parameters. The results of this study show, in agreement with current research, that the intraoperative administration of catecholamines can be used as an important anesthesiological instrument for circulatory stabilization in the case of free flap transplants after tumor resection in oral and maxillofacial surgery.de
