dc.description.abstractgerDie extrakorporale Photopherese (ECP) ist eine weit verbreitete und nebenwirkungsarme Zweitlinientherapie der Graft-versus-Host Disease (GvHD), einer der Hauptgründe für die hohe Morbidität und Mortalität nach hämatopoetischer Stammzelltransplantation. Auf Grund von patientenspezifischen und technischen Limitationen in der praktischen Umsetzung dieser Therapiemethode kann sie allerdings bei einigen Patienten nicht angewandt werden. Bei einer erfolgreichen Modifikation des Therapieverfahrens mit Spenderzellen könnten mehr Patienten von dieser Therapie profitieren. In der vorliegenden Arbeit wurde eine allogene extrakorporale Photopherese in einem Mausmodell der akuten Graft-versus-Host Disease mit Leukozyten eines genetisch inkompatiblen Spenderstamms sowohl zum ursprünglichen Stammzellspenderstamm als auch zum Empfängerstamm durchgeführt. Zudem wurde eine mixed lymphocyte reaction (MLR) zur Messung relevanter Zytokine (IL-2, IL-10, TNF-α) durchgeführt und analysiert, inwieweit sich diese Methode zur weiteren Forschung im Rahmen der allogenen extrakorporalen Photopheres eignet. Es konnte gezeigt werden, dass die Therapie mittels allogener extrakorporale Photopherese in zwei Versuchsgruppen je mit Spenderzellen eines genetisch inkompatiblen Spenderstamms keinen signifikanten Effekt auf das Überleben oder die Krankheitslast der Versuchstiere im Vergleich zu einer Placebogruppe hatten. Es zeigten sich keine statistisch signifikanten Unterschiede in den gemessenen Anteilen der T-Helferzellen (CD3+CD4+), den zytotoxischen T-Zellen (CD3+CD8+), den NK-T-Zellen (CD3+CD49b+) und den regulatorischen T-Zellen (CD4+CD25+FoxP3+) bezogen auf die unterschiedlichen Therapiegruppen. Allerdings konnte ein signifikanter Unterschied im Anteil der NK-Zellen (CD3-CD49+) der bis zum Versuchsende überlebenden Tiere im Vergleich zu den vorzeitig verstorbenen Versuchstieren unabhängig von ihrer Zugehörigkeit zu den Therapiegruppen festgestellt werden. In der mixed lymphocyte reaction zeigten sich keine signifikanten Unterschiede in den gemessenen Zytokinen. Abschließend kann durch eine detaillierte Analyse und Einordnung der erhobenen Ergebnisse in den aktuellen Stand der Forschung aufgezeigt werden, wie und warum weiterführende Untersuchungen zum genauen Mechanismus der extrakorporalen Photopherese benötigt werden, um den Weg für eine allogene extrakorporale Photopherese am Patienten zu ebnen.de
dc.description.abstractengExtracorporeal photopheresis (ECP) is a well-established and well tolerated second line therapy in graft-versus-host disease (GvHD), a major cause of high morbidity and mortality after a hematopoietic stem cell transplantation. Due to technical restrictions not every patient can be treated with extracorporeal photopheresis so that a successful modification of this treatment using donor cells could give more patients access to the treatment. Therefore, in this thesis an allogeneic extracorporeal photopheresis in a mouse model of acute graft-versus-host disease was conducted with leukocytes from donors (mice) that had a genetic background different to the initial donor of the stem cell as well as the treated animal. In addition, a mixed lymphocyte reaction (MLR) was conducted to measure different relevant cytokines (IL-2, IL-10, TNF-α) and the suitability of this method for further research concerning the impact of allogeneic extracorporeal photopheresis was examined. It was shown that the two test series of allogeneic extracorporeal photopheresis with donor cells of different genetic backgrounds had no significant effect in comparison to a placebo group on the survival or the disease burden. There were no statistically significant differences in the percentages of helper T cells (CD3+CD4+), cytotoxic T cells (CD3+CD8+), NKT cells (CD3+CD49b+) and regulatory T cells (CD4+CD25+FoxP3+) measured relating to the different treatments. However, it was shown that NK cells (CD3-CD49+) in general differed significantly from the individuals that survived the test series duration in contrast to those that died during the experiment. In the mixed lymphocyte reaction, no significant differences were seen in the cytokines measured. Finally, the detailed analysis of the results and their comparison with the current knowledge frontier show how and why more research concerning the exact mechanism of extracorporeal photopheresis is needed to pave the way for allogeneic extracorporeal photopheresis.de
